After receiving support from the local community and state, OCHC applied for and received New Access Point funding in June 2012. Since that time, OCHC has strategically grown and expanded to address health care needs of the service area. According to 2023 UDS data, OCHC served 10,438 patients at 14 service delivery sites, including five free-standing clinics (three are dental), nine school-based clinics (mental health services), and a mobile dental van. OCHC serves the four-county service area of Dade, Dallas, Hickory, and Polk counties in Missouri. These counties are all rural with a combined population of 65,085. All counties have Medically Underserved Area designations, and all counties have low-income population Health Professional Shortage Area designations for primary care, dental, and behavioral health. The target population for the Service Area Competition includes 26,884 low-income residents (living at or below 200% of the federal poverty guidelines), according to U.S. Census estimates. OCHC provides quality health care services utilizing a sliding fee discount schedule, regardless of insurance status or ability to pay, eliminating financial barriers facing members of the target population. OCHC utilizes an evidence-based model of care known as the patient-centered medical home (PCMH) model. OCHC is recognized by the National Committee on Quality Assurance as a PCMH, indicating OCHC utilizes a team-based approach focused on whole-person wellness and patient engagement. The mission of OCHC is to provide quality, compassionate, and professional healthcare to all. How the Project Will Address Need for Health Care and Proposed Patients / Providers: Service Area Competition funds will be used to continue providing comprehensive health care services, including primary and preventive medical, dental, vision, and behavioral health care. Service site locations will include five free standing clinics (three are dental), nine school-based clinics (mental health services), and a mobile dental van. Staffing at these clinic locations will include 9.58 FTE medical providers (1.98 FTE contracted staff with Citizens Memorial Hospital), 10.2 FTE dental providers (5.85 FTE dentists and 4.35 FTE dental hygienists), 0.1 FTE contracted optometrist, and 9.2 FTE behavioral health providers (1.15 FTE contracted staff with Comprehensive Wellness Clinic) to serve an estimated 10,438 unduplicated patients by the end of December 2026.
