Health Center Program - Health Center Cluster - Website Address: www.adaptoregon.org Grant Funds Requested: 1,932,433 Needs to be Addressed Adapt, dba Adapt Integrated Health Care, a Federally Qualified Health Center (FQHC) in rural Douglas County, Oregon, is applying for Competing Continuation HRSA-24-070 funding to provide comprehensive and fully integrated primary care and behavioral health care to low-income and medically underserved patients, regardless of their ability to pay. Residents of rural Douglas County are more likely to be classified as low-income, uninsured, unhealthy and medically underserved than their urban counterparts. Our rural population has a higher rate of chronic disease, mental illness, and substance use disorders, higher mortality, and lower life expectancy than residents statewideâ€”straining families, health systems, social support services, and communities. Douglas County is ranked among the least healthy counties in Oregon. Nearly 60% of low-income adults in Douglas County have at least one costly and preventable chronic conditionâ€”arthritis, asthma, diabetes, cancer and heart disease among the most common. Data for our defined service area show a total population of 87,341 residents, of which 30,696 (35%) are low-income. While an estimated 25% of our service areaâ€™s population is covered by the Oregon Health Plan (Medicaid), 29% of our service areaâ€™s adult population has no usual source of care and 12% have delayed or not sought care due to cost. Proposed Services The proposed project will provide comprehensive and fully integrated primary care, behavioral health care, mental health care, addiction treatment and prevention services to low-income, homeless and medically underserved patients, regardless of ability to pay. Other services, such as hospitalization, prenatal care and dental care, are provided through established arrangements with our local rural hospital, CHI Mercy Health, and Aviva Health, a Federally Qualified Health Center partner organization. Adapt was founded in 1971 as an Oregon not-for-profit for the sole purpose of providing prevention and treatment of substance use disorders. The organization has grown over the years in response to community need. In 2006, Adapt opened its first primary care practice to address the high degree of overlap between addictive disorders, mental illness, and unaddressed health care needs. The practice was certified as a participating Rural Health Clinic in 2008, and in 2012 achieved Federally Qualified Health Center status under the Community Health Center (CHC) and Health Care for the Homeless (HCH) categories to deliver comprehensive, culturally competent, high-quality primary care, behavioral health care and support services, regardless of the patientâ€™s circumstances or ability to pay. Service Delivery Sites & Population Served Adapt operates five service delivery sites in rural Douglas County, offering integrated primary care, behavioral health care, mental health care and substance use treatmentâ€”creating a no-wrong-door approach to patient care. In 2022, Adapt provided 53,408 in-person and telehealth visits to 7,494 unduplicated patients. A total of 742 patients self-identified as homeless or at risk of homelessness. Our proposed target patient population includes residents of Douglas County who reside in the Adapt Service Area, prioritizing those who are low-income, medically underserved and/or homeless. Adapt proposes to serve 8,066 unduplicated patients in year 2025.
