The Health Center Sites now provides comprehensive primary care, oral health, mental health, and substance abuse services in an integrated, interdisciplinary care model. Enabling services provided include Health Education, Chronic Disease Self-Management, Tobacco Cessation, STD/HIV Prevention and Counseling. The Health Center coordinates closely with the Public Health Department programs including the WIC Program Services, Family Home Visiting, Child Abuse and Prevention, Domestic Violence Victim Services, Child and Adult Immunizations, Public Transportation, Environmental Services, Chronic Disease Screening and Public Health Nursing Services. t Tarro, CEO/Public Health Administrator, leads a team of 160 staff and manages an annual budget of more than $9,500,000. The Maple Street Clinic currently serves more than 7,700 residents annually, providing more than 40,000 visits annually. The target population for the Health Center Sites include uninsured and underinsured citizens, Medicaid/Medicare insured, children 3 to 19 years old, and Public Housing residents, which together comprise 55% of Macoupin County residents. Macoupin County is MUA/MUP designated, a primary care and mental health shortage area, and a low-income dental shortage area. Major healthcare needs include cardiovascular disease, diabetes, oral and behavioral health access and substance use disorder treatment and recovery services. Healthcare barriers include the cost of care for low-income residents and lack of transportation. Through continued CHC/PHPC funding, MCPHD will increase by over 8% the number of residents served in rural Macoupin County and surrounding areas. Our plans include increasing outreach visits for medical and behavioral care liaising with public health services and community organizations to reduce barriers to care for our patients The SAC grant will enable MCPHD to employ 3.0 FTE Family Nurse Practitioners, 1.0 FTE Physician Assistants, 3.0 FTE Psychiatric Nurse Practitioners, 4.0 FTE LCSW’s, 3.0 LCPC’s, 7.0 FTE dentists, 4.2 FTE Dental Hygienists and 13.0 FTE Expanded Function Dental Assistants. Service levels will include: 2,900 patients (9,500 encounters) for medical, 6,000 (18,500 encounters) for dental and 1,500 mental health and substance use disorder patients (19,000 encounters). MCPHD will provide services to 9,000 unduplicated residents over 47,000 encounters annually at full operational capacity.
