Health Center Program - Project Abstract: HRSA Service Area Competition (SAC) Applicant Organization: Mt. Enterprise Community Health Clinic dba Crossroads Family Care Project Title: Expand and sustain the delivery of Medical, Dental, Behavioral Health, and Health Education services. Service Area ID: 082 Funding Request: 2,500,000.000 annually Project Period: 2/28/2026-2/28/2030 Overview: Mt. Enterprise Community Health Clinic dba Crossroads Family Care, a Federally Qualified Health Care Center established in 2003, serving residents of Rusk County and Anderson County, with four locations, providing care for approximately 7,800 patients annually, respectfully submits the Service Area Competition Grant to request continued HRSA funding. The funding will develop and operate a mobile clinic to increase access to comprehensive primary health care, dental services, and behavioral health services for low-income, underserved, and medically vulnerable Rural populations, The proposed Mobile Medical Clinic project will deliver integrated primary medical care, behavioral health services, preventive screenings, dental services, health education, and medication distribution in conjunction with the on-site 340B pharmacy. The clinic will improve health outcomes, reduce preventable emergency room visits, and increase access while addressing non-medical drivers of health. The mobile unit will be equipped with telecommunication services, enabling telehealth services, promoting electronic access, and improving accessibility. It will also provide the ability to educate patients on using mobile apps and web services for patient portals, allowing them to access their records, message clinic staff, receive medical and clinic updates, and receive health education. Additionally, eligibility specialists serving as patient navigators will be available to assist qualified residents with enrollment in sliding fee discount services, marketplace insurance, and other support services. Service Provided: Medical Services: Adult and Pediatric Primary care, preventative screenings, onsite laboratory, chronic disease management, Women's Health, OB services Q2 2026. Dental Services: Exams, cleaning, fillings, extractions, dentures, fluoride treatments, oral health education. Behavioral Health: Juvenile, Adolescent, Adult, and Couple counseling. Veterans, trauma, PTSD, Diagnostic screenings for ADHD, ADD, and medication Management. Pharmacy: 340 B Contracted Pharmacies, Onsite Pharmacy Q2 2026 Support Services: Referrals, an integrated care network with specialists and hospitals, insurance enrollment assistance, and health education trainings. Statement of need: Over 40% of the population is uninsured or underinsured, with a high population of Hispanic and African Americans with limited access to affordable care. Barriers such as language, lack of education, including health education, transportation, and lack of income are common in this service area. Bringing care directly to the rural communities with a mobile dental clinic will address and overcome barriers. Target Population: Rural participants who are low-income or below 200% of the Federal Poverty Level. Mobile Unit : A fully equipped mobile medical, dental clinic with one exam room, telehealth area, one dental suite, lab area, reception area for check-in, eligibility/enrollment services, and ADA accessibility. Mobile Staffing: - Medical Provider NP or PA - Dentist - Dental Hygienist - Dental Assistant -Medical Assistant/Nurse -Driver/ Office Support Goals and Objectives: 1. Provide an additional 300 medical and 100 dental visits within the first year, with increases expected each year of the service period to meet or exceed 10,000 visits. 2. Reduce preventable ER visits for primary and dental care issues. 3. Deliver health/dental education to at least 1000 community members annually. 4. Increase dental care usage by 10% annually among the target population. 5. Maintain access to current patients served.
