(LHP) was incorporated as a 501c(3) nonprofit organization in 1998 and was part of Copley Health Systems; a network that included a critical access hospital. Subsequently, LHP became an independent nonprofit organization in 2005. In March of 2006 LHP became a FQHC-look-alike. Since 2006, Lamoille Health Partners has been providing comprehensive, quality, and affordable healthcare services and programs to the people of Lamoille County. As a non-profit health and wellness provider, LHP achieved Federally Qualified Health Center (FQHC) status in 2008, designating the organization as the only FQHC in Lamoille County. That year, LHP also entered into a joint venture to open the Community Health Pharmacy, giving patients access to mail order prescription medications at a low price. In 2010, LHP added dentistry to the service offerings, followed by pediatrics in 2012. As part of a major expansion of services, LHP opened the Lamoille Health Pharmacy retail location in Stowe in 2022, providing fills and refills as well as medication support to the entire community, helping even more people save money on their prescription medicines. In 2022, LHP achieved a remarkable HRSA Operational Site Visit under the HRSA Health Center Compliance Manual requirements. LHP has become a respected and vital patient medical home throughout the service area, including with the medically underserved and uninsured residents. The focus of LHPâ€™s service strategy is on decreasing health disparities, providing critically essential health care services, increasing access, and eliminating barriers to comprehensive care. LHP focuses on reaching out to families using a community-family orientation that assures the provision of comprehensive and integrated primary services covering all life cycles, thus optimizing continuity of care within a familiar and stable system. Today, more than 19,000 members of the community trust Lamoille Health Partners with their healthcare needs. Lamoille County, located in mountainous northern Vermont, has a population of 26,090 and is one of the fastest growing counties in Vermont. The county has a total area of 462.25 square miles. Lamoille County is rural with a population density of 56.1 people per square mile. Additional geography details are shared throughout this section. LHP serves Lamoille County, including the following communities and villages: Belvidere, Cambridge, Eden, Eden Mills, Elmore, Hyde Park, Jeffersonville, Johnson, Morristown, Waterville, Wolcott, and Stowe; with high-quality low-cost comprehensive medical services for all life cycles, from pediatrics to geriatric care. Service locations include the Lamoille Health Family Medicine - Morrisville, Lamoille Health Family Dentistry, Lamoille Health Behavioral Health and Wellness, Lamoille Health Family Medicine â€“ Stowe, and Johnson Elementary School. LHP is also in the process of adding a new service site â€“ Lamoille Health Family Medicine â€“ Cambridge. LHP provides medical, dental, behavioral health and substance use services, 340B pharmacy services, tele-health/behavioral health, express care as well as a cadre of enabling services to the residents of Lamoille County; the LHP target population. In addition, LHP is awaiting the delivery of a mobile dental van to increase access for the service area population.
