CSCHC has since grown to include service sites in contiguous Morrow and Crawford counties. The CSCHC mission is to improve community wellness through neighborhood access to quality, affordable, integrated primary healthcare. Its goal is to offer a comprehensive range of healthcare services on a sliding-fee scale to all community residents regardless of income or insurance coverage, no one is denied service. CSCHC offers a full range of services at each of its health centers and has achieved formal NCQA - PCMH Recognition. CSCHC is committed to Population Health Management to achieve a healthier community and has consistently had steady year over year growth since its beginning. NEED: The primary service area for this SAC Project is six (6) contiguous zip codes, with a population of approximately 92,598, encompassing parts of the contiguous Counties of Marion, Morrow, and Crawford. This proposal, by the Center Street Community Health Center (CSCHC) is to continue to bring quality, easily accessible and comprehensive primary health care to the areas’ indigent residents. All 3 counties are predominantly rural and have currently designated Medically Underserved Areas (MUA’s). Poverty and unemployment continue to negatively impact residents’ health. Poverty rates in the service area have decreased but remain above state and national levels. According to the current Geo Navigator data, these 6 zip codes are home to 92,598 persons with 31,680 of them having incomes below 200% of poverty and 10,771 of those having incomes below 100% of poverty. Furthermore, UDS Mapper indicates 5,973 persons in the service area have no form of health insurance. RESPONSE TO NEED: CSCHC, is a safety-net healthcare provider with three (3) permanent community health centers all of which provide “one stop” to receive medical, oral, and mental health services. CSCHC maintains a team of physicians, nurse practitioners, dentists, and mental health providers with which it addresses health care disparities. Core medical, behavioral, and dental services were provided by 16.5 FTE licensed professionals. CSCHC is the predominate health center in all 3 counties. CURRENT NUMBERS AND PROJECTED SERVICES: In 2024 CSCHC provided over 17,000 medical encounters, 6,500 dental encounters and 14,000 behavioral health encounters to 10,157 users. For 2026-30, CSCHC plans to continue expanding primary care, behavioral health, and oral health services utilizing a coordinated patient-centered medical home model to ensure access to comprehensive healthcare for those less fortunate in the service area.
