Health Center Program - Health Center Cluster - Service Area ID Number: 319, Beulah, ND Project Title: Service Area Competition Applicant Name: Coal Country Community Health Center Website Address: www.coalcountryhealth.com Grant Program Funds Requested: $2,750,316 Number of patients to be served in 2025: 9,575 Coal Country Community Health Center (CCCHC) is a not-for-profit community health center organization located in North Dakota. The organization incorporated in 2003 and received Community Health Center funding in 2004. The CCCHC service area consists of over 4,500 square miles and is comprised of all of Dunn, Mercer, and Oliver Counties, the southern portion of McKenzie County, and the greater part of Morton County in west central North Dakota. In addition to its administrative offices, CCCHC maintains nine health center delivery sites, including four comprehensive health centers, and five school-based clinics in the communities of Beulah, Hazen, and Killdeer. CCCHC staff provides health services to its populations across the lifespan, including required primary and preventive medical care services, diagnostic and screening services, outreach, health education, disease management, behavioral health, substance use, enabling, and other services. CCCHC contracts with area partners to allow patients access to discounted dental and specialty care services. In 2022, CCCHC provided care to 9,026 patients through 36,813 visits. There are no other federally funded community health centers in the service area and only one other safety net provider; thus, CCCHCâ€™s role in service delivery to area residents is essential for improving regional population health. The service area population includes 47,973 persons, and the target population of 19,527 includes low-income and vulnerable populations, as well as others in the service area who have no other access to primary care. Social determinants of health play a key role in the health of area residents, with 23.6 percent of the population low-income, 8.5 percent uninsured, and only 25.3 percent having a college degree; in addition, 17.0 percent of the population is elderly. Service area residents face significant health disparities, health inequities, and barriers to affordable health care. Key health status indicators include high rates of mortality from cardiovascular disease, stroke, cancer, suicide, and unintentional injury. In addition, obesity, behavioral health, substance use issues, and prenatal risk behaviors all have a significant negative impact on the target population. Among elderly and vulnerable residents, co-morbidities and functional limitations further negatively impact health status. Over the next period of performance, CCCHC will continue to meet critical community needs by offering its full scope of services to all patients coming for care regardless of age, race, income, gender identity, sexual orientation, insurance status, or family size. CCCHCâ€™s interdisciplinary team will offer service area residents access to a patient centered medical neighborhood health home and holistic, integrated, high quality, affordable health services. The CCCHC team will continue its commitment to improving health outcomes, reducing health disparities, encouraging prevention and wellness, and promoting community collaborations aimed at maximizing available resources and enhancing the lives of service area residents.
