Health Center Program - HEALTH CENTER PROGRAM: H80CS09088 SERVICE AREA ID NUMBER, CITY, AND STATE: ID # 208; Charleston, SC PATIENTS PROJECTED TO BE SERVED IN 2026: From 01/01/2026-12/31/2026, Fetter Health Care Network, Inc. (FHCN) projects serving 19,883 total patients. This represents 95% of the 20,929 patient target as noted in the SAAT, with no reduction in federal funds per SAC guidance. This projection includes 800 Migratory and Seasonal Agricultural Workers (MSAW) and 1,000 persons experiencing homelessness. DESCRIPTION OF PROJECT: FHCN, a non-profit community health center, has been providing accessible, patient-focused primary health care to residents of Berkeley, Charleston, Colleton, and Dorchester counties of SC since 1975. In 2023, FHCN served 20,929 patients with 38,450 medical, 3,148 dental, 1,658 mental health (MH), 3,148 substance use disorder (SUD), 1,021 vision, and 2,512 enabling encounters. FHCN also receives Migrant Health Center (MHC) funding to provide targeted services to MSAW and Healthcare for the Homeless (HCH) funding to serve persons experiencing homelessness. In 2023, FHCN served 827 MSAW patients and 1,024 HCH patients (UDS). FHCN has received Patient Centered Medical Home (PCMH) recognition with Distinction in Behavioral Health Integration from the National Committee on Quality Assurance (NCQA). As an existing FQHC with an established presence, FHCN is well-positioned to continue providing access to comprehensive primary and preventive health services for low-income residents of the service area. NEEDS TO BE ADDRESSED: Data from the service area reveal systemic, widespread health disparities with a need for comprehensive primary and preventive health services accessible to those with low incomes and the uninsured. The population to provider ratio in the service area shows a severe shortage of primary care, dental, and mental health providers, presenting a significant barrier to care, particularly for low-income residents. Among FHCN patients, 58.1% live with incomes at or below 200% FPG and over a third of patients are uninsured (UDS 2023). All four service area counties are designated Health Professional Shortage Areas (HPSA) and Medically Underserved Areas (MUA) or Populations (MUP). FHCN’s special populations face unique barriers to accessing healthcare. Both MSAW and HCH are vulnerable populations that face a wide array of obstacles to optimal health, with homeless individuals having significantly poorer health than their housed counterparts (Rural Migrant Health, Rural Health Info Hub, accessed 1/23) (“Homelessness & Health: What’s the Connection?” NHCHC, 2019). PROPOSED SERVICES: FHCN operates 30 active sites, including 7 school-based clinics, 5 mobile units, 3 administrative sites, and 3 seasonal locations that serve MSAW; one additional Vision Center site is anticipated to open in early 2025. Services provided directly or by referral include primary care, lab, radiology, screening, coverage for emergencies, family planning, immunizations, well child, gynecology, obstetrics, preventive and additional dental, pharmacy, HCH-required SUD, case management, eligibility assistance, health education, outreach, transportation, translation, mental health, nutrition, ophthalmology, podiatry, and psychiatry services. Proposed staffing to support the project include 9.2 FTE physicians, 12.4 FTE physician extenders, 2.0 FTE nurses, 32.0 FTE medical assistants, 10.0 FTE lab staff, 2.0 FTE dentists, 1.6 FTE hygienists, 3.45 FTE LISW/LPCs, 1.0 FTE psychiatric nurse practitioners, 0.75 FTE SUD providers, 1.0 FTE ophthalmologist, 1.0 FTE optician tech, 10.4 FTE pharmacy staff, and 11.6 FTE enabling staff. POPULATION TO BE SERVED: FHCN prioritizes serving low-income individuals in the service area, including Medicaid recipients and the underinsured/uninsured. FHCN also serves MSAW and persons experiencing homelessness.
