Health Center Program - GraceMed Health Clinic, Inc. (“GraceMed”), a competing continuation applicant, Health Center Program award number H80CS08772, proposes to serve 50,368 unduplicated patients in the calendar year 2026 in the service area ID number 264 –Wichita, Kansas. Our target population are those experiencing poverty, homelessness, untreated chronic illness including mental health and substance use disorders, homelessness, immigration status, or sudden loss due to a personal disaster, crime, or other life-altering event, and especially those who are uninsured or under-insured. We operate 16 clinics in the urban cities of Wichita and Topeka, and rural towns of McPherson and Clearwater. A recognizable proportion of residents in each of our communities face multiple barriers in accessing primary health care because of their individual and family situations, with many still unable to qualify for affordable coverage under ACA, government health coverage programs (CHIP/Medicaid), along with growing scarcity of primary health care providers (particularly in rural areas) willing to accept uninsured patients. Our designated service area is designated as a primary and behavioral health professional shortage area by the Bureau of Health Workforce. By disease incidence, our communities experience higher rates of asthma, obesity, diabetes, high cholesterol, infant mortality, hypertension, substance abuse, and tobacco use compared to the U.S. average. The leading causes of death are heart disease, cancer, lung disease, accidents, and diabetes. Health disparities are evident given the low number of primary health care providers and high rates of preventable diseases typically treatable at a primary health care doctor’s office. GraceMed provides services for all ages and personal situations, including care for the homeless. We provide access to primary general medical care; vision and dental services; behavioral health care; podiatry, laboratory, diagnostic, and pharmaceutical services. Mental health and substance use disorder services, along with other medical specialty care, are offered through contractual arrangements. We integrate health education into all direct care plans to encourage healthy lifestyles. Case management is a vital member of the primary care medical team to ensure patients receive follow-up care and enabling referrals to community resources. GraceMed clinics are staffed with 24.5 FTE medical providers, 19.6 FTE dental providers, 2.0 FTE behavioral health providers, 2.8 FTE optometrists, 4.0 FTE pharmacists, and 1.5 FTE podiatrists. Fifteen (15) of the sixteen (16) GraceMed’s clinic sites are Patient-Centered Medical Home (PCMH) recognized, with the last site being prepared for certification. This integrated, team-based healthcare delivery model (PCMH) provides a centralized setting that coordinates delivery to ensure patients receive treatment when and where they need it and in a manner the patient clearly understands and successfully complete. Continued funding is critical to further our mission in providing high-quality, comprehensive healthcare to many of the most vulnerable living in Kansas. This support ensures our ability to maintain not only quality services but preserves access for all and our ongoing reach of care for those living in and around our Kansas communities.
