Crescent Community Health Center (CCHC) first received Health Center Program funding in 2007 to provide care for the underserved in Dubuque County in northeast Iowa. For 18 years, CCHC’s impact in the region has expanded to serving patients from Iowa counties of Allamakee, Clayton, Delaware, and Jackson, as well as Jo Daviess County, Illinois, and Crawford, Grant, and Iowa counties in Wisconsin. The service area population is 235,728 and the target population includes 59,356 individuals with income at or below 200% of the Federal Poverty Level (FPL), the uninsured and underinsured, and those facing other barriers to care. These populations experience significant healthcare challenges including higher rates of diabetes, obesity, hypertension, heart disease, and cancer compared with the overall population. Data Source: Health Center Program GeoCare Navigator. Over 10% of patients are best served in a language other than English, primarily Spanish, Marshallese, Pohnpeian, Pushtu. Almost 87% of patient incomes are at or below 200% of FPL, 22% are uninsured, and 56% are Medicaid/CHIP recipients. CCHC provides comprehensive primary care services for all ages to meet the needs of its patients. Medical services include physicals and wellness exams, preventative care, vaccinations, school/sports physicals, acute illness treatment, minor procedures, family planning, and chronic disease management. Dental services include routine exams, hygiene screenings, fillings, extractions, root canal therapy, stainless steel crowns, dentures, deep cleanings, and oral surgery. CCHC’s mental health clinic provides integrated care for disorders including depression, anxiety, bipolar, schizophrenia, trauma, grief, and loss. Pharmacy services, including access to 340B medications, are provided through CCHC’s pharmacy. CCHC’s Quickcare offers walk-in appointments for urgent or serious, but not life-threatening conditions. Enabling and referral services are provided to ensure patients have full access to care, including, but not limited to, transportation, housing/food assistance, language interpretation/translation, and specialist referrals. CCHC maintains collaborative relationships with other area providers to ensure patients can access a full continuum of services. CCHC offers a sliding fee discount for those at or below 200% of the FPL and certified application counselors help patients determine eligibility for, and enroll in, coverage through the Health Insurance Marketplace, Medicaid, CHIP, and SHIIP. CCHC’s commitment to providing high-quality healthcare is acknowledged through recognition as a patient-centered medical home and through the following HRSA Community Health Center Quality Recognition badges (2024): Health Care Quality Leader Bronze, Addressing Social Risk Factors to Health, Advancing Health Information for Quality. Ongoing Health Center Program funding will allow CCHC to continue providing essential primary and preventive healthcare to the underserved in the tri-state area and to focus efforts on addressing the needs of the patient population. Patient projection through calendar year 2028 is 16,732 unduplicated patients, a projected growth of 4-19% each year. This steady increase highlights CCHC’s commitment to expanding services and reaching more individuals in need.
