CFCN began as a full-time medical practice in 1964, and in 1992 was certified as a Rural Health Clinic. In 1993, Lakeland Health purchased the clinic and by 2005 it became a Federally Qualified Health Center Look-a-Like. Two years later, in 2007, CFCN applied for and was granted the status of Federally Qualified Health Center (FQHC) and in 2008 it separated completely from Lakeland Health. Currently, CFCN operates two delivery sites: Cassopolis Family Clinic, located at 261 M-62 North, Cassopolis, Michigan 49031; and Niles Community Health Center, located at 1951 Oak Street, Niles, Michigan 49120. CFCN provides primary care services including adult medicine, pediatric care, gynecology, geriatric care, behavioral health, dental, pharmacy, and most recently Substance Abuse Disorder (SUD) and Medication Assisted Treatment (MAT) therapy, along with social outreach programs, complementary and alternative care, and patient and community health education. Services are provided to the underserved communities, with CFCN offering a Sliding Fee Discount Policy (SFDP) that adjusts fees based on the patient’s ability to pay. CFCN has established itself as a vital safety net provider in the primary service area, serving nearly 14,000 patients annually with over 75,000 visits. CFCN’s service area has the following Health Professional Shortage Area (HPSA) scores: Primary Care 17 out of 26, Mental Health 18 out of 26, and Dental 21 out of 26- the higher the number out of 26 reflects the greatest need. CFCN is committed to continuing providing the highest quality of care to the service area. CFCN is applying for HRSA-25-016 SAC funding to continue serving the needs that exist in the health center’s current service area as defined in the Service Area Announcement Table (SAAT) with SAC ID 274, listing over 35 zip codes. CFCN is proposing to serve the patient target of 13,822 by the end of the 3-year budget period.
