Health Center Program - Grantee: Martinsville Henry County Coalition for Health and Wellness Health Center Program Award #: H80CS08749 Service Area ID, City, and State: 061, Martinsville, VA Total Projected Unduplicated Patients for CY 2026: 7027 Martinsville Henry County Coalition for Health and Wellness (MHCCHW) DBA Connect Health and Wellness is a Federally Qualified Health Center that serves residents of all ages in Franklin, Henry, and Patrick Counties in Southcentral Virginia and Rockingham and Stokes Counties in Northcentral North Carolina. The service area spans 2,570 square miles throughout the peaks, valleys, and foothills of the Appalachian and Blue Ridge Mountains. The funding will be utilized to continue providing comprehensive healthcare that is not only responsive and accessible but also patient-centered. Our five strategically located sites throughout the service area ensure the provision of all required and some additional services: comprehensive primary healthcare; comprehensive and additional dental health services; mental health services; voluntary family planning; immunizations; coverage for emergencies during and after hours; lab and radiology services; 340B and pharmacy assistance services; case management; referral coordination; health education; translation; outreach, and eligibility assistance. These services are accessible to anyone, regardless of age, race, sex, gender, orientation, religion, ability, and culture. They are also provided regardless of ability to pay, ensuring that no one in our community is left without the care they need. Unfortunately, the service area fares more poorly than their state or country counterparts in 15 of the top 16 leading causes of death, making it one of the unhealthiest regions in the country. (Alzheimer’s Disease, Heart Disease, Malignant Neoplasms, Diabetes, Liver Disease, etc.) Some factors that impact residents’ health status are geography, provider shortages, and poverty. The service area is geographically vast, and the mountainous terrain leaves many poor residents isolated. Due to steep inclines and narrow passages made even more dangerous by winter weather, periodic flooding, and road closures, driving can be treacherous, time-consuming, and unsafe. Public transportation is lacking outside the few cities, further isolating rural residents. The ratio of physicians, dentists, mental health providers, and specialists grossly impacts the ability of residents to receive care. HRSA has identified all counties with a Health Professional Shortage Area (HPSA) in every possible area – Primary Care, Mental Health, and Dental Care. This funding, fortunately, will address these shortages. Finally, poverty is a root cause affecting health status. One in 5 children in the service area live in poverty, and the median household income in all counties is lower than state or national counterparts. Families must often decide between healthcare copays, prescription costs, healthy foods, car fuel, utility bills, childcare expenses, etc. These are difficult decisions, and – when faced with immediate needs for food and shelter – healthcare is often forgone, further exacerbating chronic and acute illness. Since 2005, MHCCHW has remained steadfast in its commitment to improving the health of its community. As a Patient-Centered Medical Home certified through the National Committee for Quality Assurance (NCQA), the Center is unwavering in its mission of improving the overall health of the community.
