Since its inception, Community Health Center of Central Missouri (CHCCMO) has continuously demonstrated its experience and expertise in developing and implementing appropriate systems of care and services to address the needs of the target population. CHCCMO has consistently responded to the needs of the underserved, working diligently to loosen budgetary constraints in an effort to meet those growing needs. CHCCMO distinguishes itself as the only comprehensive primary health care safety net delivery system in the area, successfully meeting all challenges associated with delivering care in a large geographic area spanning eighty-three (83) zip codes. These ZIP codes are predominantly located in the central Missouri counties of Callaway, Cole, Miller, Moniteau, and Osage. In 2023, CHCCMO served 15,826 patients, providing integrated medical, dental and behavioral health care services. Needs to be addressed. Priority health care needs of the target population include diabetes, cerebrovascular disease, lack of preventive screenings, low birth weight babies and behavioral health issues. CHCCMO provides access to on-going preventive care for low income and uninsured individuals. Overall, the five-county area has a higher rate of veterans, lower average household income, lower per capita income, and lower rates of individuals with advanced degrees. Proposed Services. CHCCMO provides a full scope of services, including primary, oral health, and behavioral health care. The proposed project reflects a staffing and leadership structure to assure timely, affordable care for the target population. There are currently four community-based service delivery sites located in Jefferson City, Linn, Fulton, and California, along with mobile and school-based services. Providers include physicians, nurse practitioners, dentists and hygienists, and behavioral health providers along with necessary medical, enabling and facility support staff. CHCCMO provides the full scope of care using a patient-centered medical home model. CHCCMO projects to serve 18,992 patients, exceeding the HRSA SAAT patient target of 15,826. Population Group(s) to be Served. While CHCCMO primarily serves Callaway, Cole, Miller, Moniteau, and Osage counties, the current SAAT-defined service area includes 83 zip codes in and beyond the five-counties in central Missouri. The total zip code defined service area includes a total population of 525,966 individuals while the primary five-county total population is 175,037 individuals. Of the total individuals living in the CHCCMO service area, 26.63% are living below 200% FPL. The service area also includes multiple Health Professional Shortage Areas for primary care, dental health, and mental health as well as Medically Underserved Areas for primary care in Callaway and Osage counties. CHCCMO targets the low-income population, and also fills a significant void in dental, prenatal and pediatric care services.
