In 1988, the agency became certified by the New York State Department of Health as an Article 28 Diagnostic & Treatment Center known as Community Health Center of the North Country (CHCNC). In 2007, the Canton and Malone sites were designated as Federally Qualified Health Centers (FQHCs). Gouverneur opened in 2013 and Ogdensburg opened in 2020. Community/Target Population to be served: CHCNC’s service area includes St. Lawrence and northern Franklin Counties. This is one of the most rural, isolated areas in the state, with part of the service area located in Adirondack State Park and Adirondack Mountains. In the service area, a greater percentage of individuals are living below 125 percent of the poverty level as compared to New York State, and a significant Amish population resides in the region. All towns in St Lawrence and Franklin Counties are designated either as geographic or Medicaid eligible Health Professional Shortage Areas (HPSAs) for primary care; both counties are designated as mental and dental HPSAs; and 5 communities have a Medically Underserved Area (MUA) designation. Needs/Barriers to be addressed: St. Lawrence and Franklin Counties lack enough providers who serve low-income populations. CHCNC and the hospitals’ emergency departments and clinics are the only providers that regularly provide care to significant numbers of uninsured and Medicaid patients. St Lawrence is ranked 37 and Franklin is ranked 40 out of 62 for the County Health Rankings based on health behaviors, clinical care, social and economic, and physical environment factors. Project Description: The project will continue the operation of CHCNC’s four FQHC locations: Canton, Gouverneur, Malone and Ogdensburg. CHCNC provides primary and preventative care, mental health counseling services, substance use disorder services and care coordination at all locations; dental services are available in Canton and Ogdensburg; and physical therapy and eye care in Canton. The health centers served 13,091 patients in 2023 who made 40,908 visits. CHCNC projects to meet the patient target of 13,091 patients on the Service Area Announcement Table. CHCNC has 92.13 FTE staff which includes the clinical staff of 2.6 FTE physicians, 5.8 FTE nurse practitioners, 3.25 FTE physician assistants, 0.38 FTE psychiatrist, 0.16 FTE optometrists, 1.55 FTE dentists, 1.33 FTE physical therapists, and 1.27 FTE licensed clinical social workers.
