Health Center Program - Heart City Health Center submits its competing continuation application for the U.S. Health Resources and Service Administration (HRSA), Service Area Competition funding opportunity. Established in 1993, Heart City Health is a Federally Qualified Health Center (FQHC), non-profit 501(c)3, providing primary care to the residents of Elkhart County, Indiana. The health center is funded through HRSA operating grant #H80CS08221 and its Service Area Identification Number (SAC ID) is #258. The amount requested through this application, found in the FY25 SAC Service Area Announcement Table (SAAT), is $2,552,160. With funding, the health center will achieve the patient target, set by HRSA, and projects to serve 16,206 or more patients in calendar year 2026. For over 31 years, the health center has been providing affordable, high quality medical, obstetrics, dental, behavioral health, pharmacy, and enabling services. Heart City’s target population is low-income, uninsured and underinsured individuals, including people covered by Medicaid and Medicare. It’s most recent Uniform Data System (UDS) Report provides that, in 2023, Heart City Health served a total of 16,206 unduplicated patients representing 54,484 visits to the health center, of which 86% received medical care, 19% received dental care, and 7% received mental health care. Of its patient population, fifty-seven percent (57%) live in poverty, and seventy-eight percent (78%) live at or below two hundred percent (200%) of poverty; nineteen percent (19%) have no health insurance, and fifty-six percent (56%) are covered by Medicaid. Thirty-one percent (31%) of the patient population are best served in a language other than English. There is much need for Heart City Health to continue providing health care within its designated service area. Elkhart County is designated as a Health Professional Shortage Area (HPSA) for primary medical, dental, and mental health care. Additionally, the rate of uninsurance in Elkhart County is higher for both adults and children than the national rates. Nationally, eleven percent (11%) of Americans between the ages of 18 and 64 were uninsured, while only four percent (4%) of children, 18 and below, in America were uninsured. In Elkhart County, seventeen percent (17%) of adults and twelve percent (12%) of children are uninsured. While Elkhart County residents have a similar life expectancy to the rest of the country, it experiences much greater rates of infant mortality, maternal mortality, smoking and teen births. According to CDC data, Indiana has the 7th highest infant mortality rate and the 3rd highest maternal mortality rate in the country. Twenty-three percent (23%) of adults smoke compared to 15% nationally and teen births are almost double the national rate at 28 per 100,000 to 17 per 100,000 respectively. The need for Heart City Health to continue its existing services provision, with HRSA support, is evident.
