(CHC) is a Tulsa, Oklahoma based 501(c)3 not-for-profit organization and Federally Qualified Health Center (FQHC). Established in 2003, CHC provides affordable, accessible, and quality healthcare across all life cycles, regardless of the patientâ€™s ability to pay, while respecting the dignity and culture of the community it serves. CHC currently operates three clinic locations offering comprehensive primary care, pediatrics, obstetrics, pharmaceutical services, and preventative, restorative and emergency dental services. CHC primarily targets the rapidly expanding Hispanic and/or Latino/a community in northeast Oklahoma, and over the past two decades operating as a FQHC, has become a fundamental medical resource to individuals in Tulsa, Oklahoma and the surrounding community. According to CHCâ€™s most recent Uniform Data Systems (UDS) Report, in 2022, CHC served a total of 14,834 unique patients, 6,824 (46%) of whom were uninsured, 12,504 (84%) identified themselves as Hispanic and/or Latino/a, and 10,859 (73%) were best served in a language other than English. Over the most recent four-year project period, this equates to an 8% increase in the total number of unduplicated patients served, a 14% decrease in patients who are uninsured, and a 5% increase in patients who prefer to be served in a language other than English. Also, as of October 7, 2023, CHC is experiencing an additional 12% growth in unduplicated patients served in 2023. This incredible increase in patients is reflective of the immense need for affordable and accessible healthcare services in northeast Oklahoma and the importance of CHC continuing to provide services as a FQHC. This substantial growth in patients served can be attributed to CHCâ€™s commitment to providing quality healthcare services. Of the eighteen UDS clinical measures CHC reports annually, in 2022, CHC exceeded both national and state averages on eleven measures and exceeded state averages for Oklahoma on three more measures. Additionally, CHC is able to provide quality services at a reasonable cost, with a total cost per total patient at nearly half the national average (CHC: $803.37, US: $1,339.02, OK: $1,076.73). Thus, in order for CHC to continue providing affordable, accessible, and quality healthcare services to the uninsured and underserved in Tulsa, Oklahoma and surrounding areas, CHC is seeking to maintain its status as a FQHC and be granted an additional three-year project period. If granted an additional three-year project period, CHC proposes it will serve the required 8,672 projected patient target, which equates to approximately 26,016 visits per year. To do so, CHC will continue offering primary care, pediatrics, obstetrics, dental, behavioral health, and pharmaceutical services to the community. Additionally, CHC will continue offering services at three clinic locations and hopes to continue expanding its service sites as demand continues to increase.
