(SHEF) was founded in 1979 as a community based, non-profit family medicine residency program. Salina Cares, a free clinic to serve the low income uninsured, was founded in 1997. Providers from SHEF began staffing Salina Cares in 1998. The organizations merged in 2004 to form Salina Family Healthcare Center (SFHC), which became a Federally Qualified Health Center on December 1, 2004. In January 2013, SFHC achieved recognition as a Level 3 Patient-Centered Medical Homeâ„¢ and has successfully maintained certification in the years since. Saline County is home to 53,596 people, of which the majority of residents are white (48,236) and the second largest racial/ethnic group is Hispanic/Latino (6,646). The target population is the Saline County residents living below 200% of the Federal Poverty Level (17,151). A wide range of health care needs will be addressed by the proposed project: diabetes, cardiovascular disease, cancer, prenatal and perinatal health issues, and child health. Barriers to health care include a lack of primary care physicians serving the target population and the percent of the population living below 200% of the Federal Poverty Level. The proposed project will address these needs and barriers by continuing the SFHC policy of accepting ALL patients regardless of their ability to pay, utilizing a financial screening program to assist individuals in qualifying for reduced fees, providing access to a Medicat ion Assistance Program, and by providing translation services on-site. In 2022, SFHC served 10,531 patients with 39,001 visits. This was achieved with a provider staff that included 19.65 FTE medical providers, 6.62 FTE dental providers, 0.22 FTE optometry providers, and 3.32 FTE behavioral health providers. It is anticipated that in Calendar Year 2025, SFHC will serve 10,997 unduplicated patients with 41,773 visits. This will be accomplished with a provider staff that includes 13.4 medical providers, 7.14 dental providers, 1.0 FTE optometry provider, and 2.5 FTEs behavioral health providers. A wide range of services are currently provided at Salina Family Healthcare Center, 651 East Prescott Ave, Salina, KS, 67401: preventative health care; chronic disease management; acute/urgent/emergent care; family planning; immunizations; obstetrics, including prenatal care and delivery; gynecologic care; pediatrics; geriatrics; chronic anti-coagulation management; clinical pharmacy services; in-house diagnostic laboratory and digital radiology; preventative and restorative dental services; and dental emergent care. These services will be continued in the next three program years. In addition, Salina Family Eye Care Center, 2676 Planet Avenue, Salina, KS, 67401 opened its doors in May 2022 and expanded to full-time hours in August 2023 with the hiring of a full-time optometrist. The eye care clinic provides comprehensive vision examinations and personalized optometric services including diagnosis and treatment of a wide array of eye diseases and conditions. An on-site full service optical dispensary for eyeglasses, contact lenses, and sunglasses is also available. SHEFâ€™s community outreach staff operates the organizationâ€™s two mobile medical units throughout Saline County, offering health screenings, education classes, immunizations, care coordination, and provider visits. The goal of SHEFâ€™s mobile health services is to be an extension of the care and services of fered in-house.
