Health Center Program - The mission of Rogue Community Health (RCH) is to improve health for all, especially for the most vulnerable, in partnership with our community. RCH has been providing healthcare for medically underserved and vulnerable populations in the area since 1972. RCH is a healthcare home for thousands of Oregon residents through its provision of core services that consist of primary care, preventative services (e.g., wellness visits, tests and screenings, diabetes related services, shots and vaccines), behavioral health, dental, pharmacy, and pediatric health care services. Additionally, RCH offers transitional/recovery housing, integrative health, community outreach, case management, resource navigation, eligibility assistance including OHP (Medicaid) enrollment, and school-based health and dental services. RCH operates Clinics in Medford, Ashland, White City, Eagle Point, and Butte Falls. We have Dental Services at two sites in Medford and one in Eagle Point. Members can receive whole-person well-being care through a coordinated team approach that combines medical, dental, and behavioral health care. RCH is continuously implementing progressive and evidence-based initiatives to address the core needs and help overcome barriers to access for Jackson County residents in need of healthcare services, to make primary health care accessible to vulnerable populations. RCH’s service area is within Jackson County which has a population of 220,768. The target population for this SAC is comprised of the estimated 71,340 persons in Jackson County (32.7%) who live at or below 200% of the federal poverty index. Within our service area in Jackson County, residents face multiple barriers to meeting their healthcare needs. The homeless face the most challenges. There are sparse medical and affordable housing resources, reduced healthcare accessibility due to inadequate/nonexistent public transportation, poor road quality in mountainous areas, and severe winter weather conditions. Many of the affordable homes were destroyed during major wildfires in 2020 and not replaced. This has created a long-term displacement for many vulnerable citizens. The COVID-19 pandemic impacted this same population base, and these impacted residents are slowly coming back from the setbacks suffered during this difficult period in 2020-2022. RCH is committed to fostering systemic equity in all our services, practices, and policies. RCH is dedicated to ensuring that every patient and RCH team member can pursue health and well-being free of barriers or disadvantages due to factors like race, ethnicity, language, disability, gender, gender identity, sexual orientation, social class, economic status or other socially determined circumstances. In 2023, RCH served a total of 12,816 unduplicated patients in our service area. We anticipate an increase of approximately 3% to 13,200 in total unduplicated patients served annually by the end of the period on 3/31/2028. We believe this is possible due in part to our ongoing expansion of our provision of Behavioral Health Services through increased service provision sites and increased number of specialty providers. The decrease in total patients seen after 2020 appears to be on a consistent upward trend. With an annual SAC award of $2,779,698, RCH projects that when fully staffed we will support in part the following full time equivalent (FTE) Professional positions: 9.5 Physicians; 7 Nurse Practitioners; 5 Physician Assistants; 5.5 Dentists; and 6 Pharmacists. RCH proposes annually serving 13,200 unduplicated patients of which we anticipate 325 homeless patients by the end of the budget period (3/31/2028). RCH proposes 3.5 average number of visits per patient, for 46,200 billable visits by the end of the budget period.
