VNA began providing comprehensive primary medical care for patients in 2000. In 2002, VNA was recognized as a Federally Qualified Health Center and was funded as a Community Health Center by the Bureau of Primary Health Care. In 2012, VNA changed its corporate name to VNA Health Care. VNA has sixteen permanent health center locations within its Scope of Project located in Illinois (Kane, DuPage, Will, and Cook counties): four in Aurora, four in Elgin, one in Bensenville, two in Carol Stream, and one in Wheaton, Bolingbrook, Romeoville, and Joliet. VNA also provides services in a mobile clinic. VNA is accredited by the Joint Commission as a Primary Care Medical Home. In addition to 330 and Primary Care Medical Home funding, VNA has received funding support from the Health Resources and Services Administration for projects that include New Access Points in Bensenville, Romeoville, and Joliet; increased volume; outreach and education; mental health expansion and integration; substance use disorder; pharmacy, and dental services. The VNA target population is underserved, low-income children and adults who are at increased risk of health disparities. Special populations include targeted healthcare for the homeless. Major needs among the target populations include access to comprehensive primary care, dental and mental health services, chronic disease management, and care coordination. In addition to these needs, the homeless population has an additional need for stable and safe shelter. Barriers to care include poverty, lack of transportation, linguistic isolation (VNA serves a significant Hispanic population), and low educational and health literacy. The homeless population faces these barriers, in addition to insecure housing. VNA is meeting these healthcare needs through the delivery of comprehensive primary care, other health services and obstetrics and gynecology, pediatric care, family practice, internal medicine, psychiatry and medical social work, medical home visits, home health, hospice, and public health programs, pharmacy, laboratory, dental care, optometry services, podiatry, and physical, occupational, and speech therapies. VNA improves access to care through bilingual service delivery, same-day, and walk-in access seven days a week with after-hours coverage, comprehensive care coordination, electronic medical records, enabling services, sliding fee scale discounts for self-pay patients, and extensive community outreach. VNA requests funding of $6,905,176 to support the continuation of comprehensive primary care services detailed above. Funding will also support culturally appropriate outreach, transportation, pharmacy, education, and enabling services. VNA is currently exceeding patient volume targets, and at the end of the project period, VNA anticipates serving 64,550 unduplicated patients and will provide an estimated 170,000 patient visits via the current 62 FTE medical providers, 2.0 dental providers, 1 optician, and 7.5 mental health providers.
