(HHS) began operating on December 10, 1991, with a mission to â€œProvide high quality health care services accessible to all.â€ HHS was designated a Federally Qualified Health Center (FQHC) on February 1, 2004, and is applying for continued funding through this Service Area Competition application. HHSâ€™s service area spans 22 ZCTAs throughout Peoria County, Illinois. There are 231,823 individuals in the service area, with a target population of 71,552 low-income, medically underserved residents in its service area â€“ specifically those who are at or below 200 percent of the Federal Poverty Level (FPL). The population by race and ethnicity (with Hispanic integrated) indicates that 77.4 percent are White, 14.1 percent are Black, 3.4 percent are Asian, and 3.6 percent are more than one race. Among all races, 4.4 percent report their ethnicity as Hispanic. The total population of racial/ethnic minorities in the service area is 25.2 percent. Major health care needs and barriers to be addressed by the proposed project: Social determinants of health including poverty, unemployment, health insurance, educational attainment, and transportation continue to be a key drivers of health status in the HHS service area. Health care needs in the service area are complex, and access to health care services can be extremely difficult for low-income and medically underserved populations. In 2022, an estimated 16.2 percent of all service area residents were not utilizing services at a primary care clinic, with 11.8 percent of adults having delayed/not sought care due to cost. The health center program penetration rate is 26.9 percent for low-income residents, indicating a substantial need for access to health care within the low-income population. How the proposed project will address the need for comprehensive primary health care services in the community and target population: HHS plays a major role as part of the â€œsafety netâ€ in Peoria County, IL currently operating seven health center service delivery sites to meet the needs of the community. HHS offers primary medical, preventive dental, behavioral health, specialty, and enabling services to individuals and families across the lifespan in a patient-centered, integrated model. Services are culturally and linguistically appropriate, high-quality, and comprehensive. The proposed project will address the need for comprehensive primary care services, integrated services, and specialty services in the community. The aim is to target the population by continuing services with the goal of increasing healthcare access, improving health outcomes, and reducing inequities and disparities as it relates to community healthcare delivery. Number of current and proposed patients, visits, providers: In 2022, HHS served 20,607 unduplicated patients through 58,795 clinic visits and 2,346 virtual visits provided by 182.72 FTE staff. HHS is applying for a continuation of Section 330 grant funding with a goal to serve 23,168 unique patients by the end of the three-year designation period, as established in the SAAT.
