Health Center Program - Grant #: H80CS02451 Service Area Identification Number: 106 City: Conroe State: Texas Total Number of unduplicated patients we project to serve in 2026: 33,500 Lone Star Family Health Center, the only federally qualified health center headquartered in Montgomery County, Texas, seeks funding from HRSA through the Service Area Competition (SAC) grant funding opportunity HRSA-25-013 to continue providing quality, affordable healthcare in our service area. Lone Star Family Health Center (LSFHC) has served as a source of quality, affordable healthcare in Montgomery County since its opening in 2002. As a 501(c) 3 non-profit, the primary federally qualified health center (FQHC) in Montgomery County, and one of few available to residents of North Harris County, LSFHC fulfills its mission to provide compassionate, affordable healthcare and prepare tomorrow’s family medicine professionals. LSFHC has locations in Conroe, Spring, Grangerland, Willis, and Huntsville to serve a large population of primarily underserved, uninsured, and underinsured patients. In 2024, we served 32,652 patients. We expect to serve even more patients in 2025 and 2026, as we plan to complete our project to renovate and expand our flagship clinical location and administrative headquarters in Conroe by the end of 2024. This project will yield an increase in patient volume across our organization. Thus, we expect to serve 33,500 patients in 2026. LSFHC provides a full spectrum of primary care needs, including family medicine, obstetrics, gynecology, pediatrics, geriatrics, dentistry, mental health, diabetes education, nutrition, eligibility, enrollment, patient navigation, case management, x-ray, ultrasounds, laboratory services, mammograms, pulmonary function testing, audiology, non-stress testing, pharmacy services, colposcopies, minor surgical procedures, and inpatient hospital care. Our Conroe location has a Women, Infants, and Children (WIC) facility on-site and a teaching kitchen. Additionally, LSFHC is a designated teaching health center, training 36 Family Medicine resident physicians under faculty physician supervision. We will uphold our provision of these services through the HRSA SAC grant. We fill a critical need, as 90% of our patients who report their income live at or below 200% of the federal poverty level. Texas has the highest uninsured rate of all states in the country at approximately 19%. At 28%, the uninsured rate among Greater Houston residents exceeds state (19%) and national (11%) averages, and this rate has been on the rise in recent years. Among the counties we serve, the uninsured rate is 22% in Harris County, 14.3% in Montgomery County, and 15% in Walker County. With funding from HRSA, we will close the healthcare coverage gap for the underserved by continuing to provide compassionate, affordable care to all who need it, regardless of their insurance status or ability to pay.
