Health Center Program - De Baca Family Practice Clinic (DBFPC) is located in Fort Sumner, NM and is a 501 C (3) nonprofit and the sole provider of patient centered medical and dental home healthcare in De Baca County, New Mexico. We provide services under HRSA grant H80CS00861 in service area 141. We project to serve 2,130 unduplicated patients in calendar year 2026. The Main Clinic was first funded as a Community Health Center in 2003 under Section 330 (e) and proposes to continue to provide access to comprehensive primary and preventative medical, dental, mental health, substance abuse and ancillary care services including lab and x-ray. In addition to the primary care services, the clinic partners with the Village of Fort Sumner to provide 24/7 emergency medical and/or transport services for our patients during and after-hours. All services are provided to people of all ages, backgrounds and life circumstances, regardless of their ability to pay. DBFPC also has a School Based Health Center (SBHC) located on the campus of Fort Sumner Schools providing services since 2006. A Mobile Unit to deliver services at schools in our serving area that do not have access to medical care in their schools or communities was added in 2023. De Baca Family Practice sites provide primary health care services in a frontier designated area of the rural Eastern New Mexico, primarily in the Village of Fort Sumner and the town of Santa Rosa, each being the only serviceable communities in their respective counties. Both De Baca and Guadalupe counties are designated Medically Underserved (MUA) and Healthcare Provider Shortage Areas (HPSA). The population of the target service area is 5,949, according to the most recent US Census estimates, with an ethnicity breakdown of 40% Hispanic and 55% Non Hispanic. The same report indicates 21% of the total population is living at or below the poverty level and 25% are over the age of 65, both higher than the State and National averages and indicative of the area’s agricultural economic base and the growing number of retirees living on fixed incomes. 12% of this population does not have any type of health insurance. Through our 330 Section 330 (e) funding we address the unmet healthcare need facing rural communities in our service area that have no access to any type of healthcare. Many of these patients travel lengthy distances to utilize the services DBFPC offers. Not only is access to care a barrier, approximately 68% percent of Clinic patients fall into medically indigent categories. DBFPC sites have open access scheduling and extended hours to accommodate patient’s schedules and provide a variety of outreach assistance programs designed to engage patients in their own healthcare improvement, particularly those with chronic conditions. All sites are Medicare and Medicaid approved and offer sliding fee discount services to qualifying patients. De Baca Family Practice Clinic is an Accredited Patient Centered Medical Home through the Accreditation Association for Ambulatory Health Care (AAAHC). Access to healthcare in De Baca and Guadalupe Counties is critical to the health and wellbeing of its citizens and is paramount to the economic stability of these two communities. We must maintain the best of level of care possible for the people in our service area. DBFPC would not exist without the support of its Section 330 (e) funding.
