Health Center Program - HEALTH CENTER PROGRAM: H80CS00848 SERVICE AREA ID NUMBER, CITY, AND STATE: ID #52; Hulbert, OK PATIENTS PROJECTED TO BE SERVED IN 2026: From 01/01/2026-12/31/2026, Northeastern Oklahoma Community Health Centers Inc. (NeoHealth) projects serving 26,025 total unduplicated patients. DESCRIPTION OF PROJECT: NeoHealth was founded in 2001, received its notice of grant award as a new start community health center in 2002, and began providing health care services in Hulbert, OK. NeoHealth now has 14 fixed, permanent service delivery sites serving Adair, Cherokee, Mayes, and Muskogee counties, OK, as well as an administrative site and a mobile medical van site, both based in Hulbert. The center is requesting Community Health Center (CHC) grant funds in this SAC application and has received Patient Centered Medical Home certification at its Hulbert sites. As an existing FQHC with an established presence in northeastern Oklahoma, NeoHealth is well-positioned to continue providing access to comprehensive primary and preventive health services for the low-income residents of its service area. In 2023, NeoHealth served 26,025 patients with 98,424 medical visits (98,418 onsite, 6 virtual), 1,139 onsite mental health visits, and 97 virtual enabling service visits (2023 UDS). The center’s growth trend for the period 2021 to 2023 is 16.85% (26,025 users in 2023 less 22,273 users in 2021, divided by 22,273 users in 2021), and the center expects growth to continue. NEEDS TO BE ADDRESSED: Data from the service area reveal systemic, widespread health disparities and emphasize the need for comprehensive primary and preventive health services that are accessible to those with low incomes and the uninsured. In the service area, 43.0% of individuals live with incomes at or below 200% of Federal Poverty Guidelines. The population to provider ratio in the service area demonstrates a severe shortage of primary care, dental, and mental health providers, presenting a significant barrier to care, particularly for low-income residents. All four service area counties are designated Health Professional Shortage Areas and Medically Underserved Areas (Find Shortage Areas, HRSA, 2024). In 2021, a fifth (19.7%) of service area residents under age 65 were uninsured (the most recent county-level single-year uninsured rates available), compared to 10.2% in the US. Among those with incomes under 200% FPG, 24.7% were uninsured, higher than the US percentage of 17.1% (Small Area Health Insurance Estimates: 2021, Census Bureau, 2023). PROPOSED SERVICES: NeoHealth has 14 fixed, permanent service delivery sites serving Adair, Cherokee, Mayes, and Muskogee counties, OK, plus an administrative site and mobile medical van site, both based in Hulbert. Services provided either directly or by referral include primary care, diagnostic labs and x-ray, screenings, emergency and after-hours coverage, voluntary family planning, immunizations, well child care, gynecology, prenatal care, labor and delivery, postpartum care, preventive and additional dental, pharmaceutical services, case management, eligibility assistance, health education, outreach, transportation, translation, mental health services, substance use disorder services, and Ear Nose and Throat (ENT) services. These services help to meet the needs of the service area’s identified lack of access. Proposed staffing to support project implementation include 16.0 FTE physicians (including 4.1 FTE OB/GYNs), 15.0 FTE nurse practitioners, 4.0 FTE physician assistants, 15.0 FTE nurses, 14.0 FTE other medical staff, 1.0 FTE Licensed Professional Counselor, 3.0 FTE pharmacy staff, 4.5 FTE case managers, 1.0 FTE outreach staff, and 0.25 FTE eligibility staff. By 12/31/2026, NeoHealth projects to exceed the patient target by serving 26,025 patients at a minimum. POPULATION GROUPS TO BE SERVED: NeoHealth prioritizes serving low-income individuals in the service area, including Medicaid recipients and the underinsured/uninsured.
