Health Center Program - 2025 SAC Project Abstract HC 8000811 Lake Superior Community Health Center 4325 Grand Avenue, Duluth, MN 55807-2730 SAAT ID: 129 Requested funding: HRSA CHC City: Duluth State: MN Total unduplicated patients to be served in CY 2026: 10,834 (95%) 11,404 (100%) For 52 years, Lake Superior Community Health Center (LSCHC) has served the low-income, uninsured, and underserved populations of the Twin Ports area of Duluth, Minnesota and Superior, Wisconsin, with a recent expansion to Carlton County Minnesota. As a Federally Qualified Health Care Center, LSCHC provides comprehensive medical, dental, behavioral health, chiropractic, and outreach services for all members of the community, regardless of income or insurance status. In 2023, LSCHC served 11,404 unique patients in 45,320 unique encounters with four main delivery locations in two states, not including 10 Douglas County schools currently in scope for onsite behavioral health. Patients were predominately from St. Louis County, MN and Douglas County, WI, which includes urban, rural, and sparsely populated areas. Over 50% of patients had household incomes below 100% of Federal Poverty Level; 66% were Medicaid/other public insurance; and 6% had no source of health insurance. LSCHC served a significantly higher percentage of patients of color (15%) compared to the community overall (8%). LSCHC is recognized as a Minnesota State Certified Health Care Home (HCH), and provides an effective, patient-centered, and team-based model of care the meets the needs of the whole individual and their families. The proposed service area includes designated Medically Underserved Areas and Health Professional Shortage Areas, and experiences a significant lack of access to care, especially for dental and behavioral health services. With a higher poverty rate and higher unemployment rate than state averages, the target population experiences substantial barriers including lack of transportation (especially from rural areas), lack of insurance, and lack of affordable housing leading to higher rates of homelessness. Key health problems in the area include high rates of chronic disease with significantly higher rates of premature death, particularly among the low-income and Native American population. There are also higher rates of binge drinking, drug overdose deaths, and suicide in St Louis County when compared to MN overall. LSCHC proposes to serve 10,834 patients in CY 2026, which is 95% of the SAAT patient target. Patients will be served by a robust team of health professionals, including 4.4 FTE medical providers, 4 FTE chiropractors, 5.7 FTE dentists, and 8 FTE behavioral health/substance abuse providers, along with outreach workers and administrative and clinical support staff - a total of 111.3 FTE. Tele-health will continue to be offered to increase access to care. All services will be provided in alignment with LSCHC’s mission to improve access to quality health care for all, with values that include excellence in clinical care, community-oriented services, collaboration, accessible health care, accountability, and intentionally welcoming care.
