Health Center Program - Community Health Center of Franklin County, Inc., H80- 0CS00806 NOFO Number: HRSA-25-013 Project Abstract Community Health Center of Franklin County, Inc. (CHCFC) was established in 1994 as an all-volunteer planning group of local health, education and faith-based communities and received its first 330 HRSA award in 1997. CHCFC’s defined service area is comprised mainly of Franklin County and North Quabbin regions, a rural area of Northwestern Massachusetts geographically isolated. The service area is made up of many small towns with limited access to healthcare and transportation. Over the past 27 years, CHCFC has built a leading health care delivery system in Franklin County that fills an important local health care niche. CHCFC is the only provider within the primary service area that provides medical, dental and behavior health services to individuals regardless of their ability to pay. Access to transportation and healthcare facilities is limited, with the CHCFC being one of the few providers serving the rural population. During calendar year 2023, CHCFC’s staff of 111.88 FTE with full-time equivalent employees provided 34,601 patient visits (1% decrease from 2022) with almost 70% from Medicare/Medicaid or uninsured and cash patients. This was down slightly from 2019 (~3% decrease) due to Covid 19 but is increasing to pre-pandemic slowly. We served 334 migratory and seasonal agricultural workers (MSAWs) and their families, 413 homeless, and 162 veterans. Financial constraints are a barrier to residents' ability to access healthcare services with out-of-pocket costs for transportation, medical appointments, and medications affecting care-seeking behavior. CHCFC’s defined service area has a population of 70,980, with low-income population and poverty levels significantly higher than state levels. The median income in Franklin County is $70,306. This is just under three-quarters of the amount of the state median in MA ($94,488) and a little less than the national average ($74,755). Approximately 15% of the population in Franklin County, MA live below the poverty line. This is higher than the state (10.4%), and national average (12.6%). Trending below state and national levels in cardiovascular, prenatal and perinatal and overall age adjusted death rate, CHCFC in the 340(b)-pharmacy program offers discounted medications to the underserved population and has prioritized outreach, enabling and education services. Transportation is offered by CHCFC and provides a crucial service for the aging and disabled population. The age of residents in the service area has continued to increase with the percentage of elderly 23.2% compared to 17% in the state, and 17.3% nationwide. Many depend on CHCFC for their healthcare, dental care, behavior health, and transportation to and from medical appointments. CHCFC is budgeting over 20,100 medical visits, 9,800 dental, and 1,500 behavior health visits in the first year of the new project period (FY25) with an FTE staff of 111. With this grant-funded project, we will provide high-quality, comprehensive primary care and outreach and enabling services for the communities in the Franklin County and North Quabbin regions. We also currently have a CIS pending to add a new site in Turner Falls, and a plan to expanded after-hour operating hours.
