Health Center Program - HEALTH CENTER CLUSTER - Project Title: Florida Community Health Centers, Inc. Service Area Competition Application Applicant Organization Name: Florida Community Health Centers, Inc. (FCHC) Website Address: www.fchcinc.org Grant program funds: Service Area Competition Application (SAC) #HRSA -22-005 Overview of Organization, Community to be served, and Target Population: Since 1976, FCHC has served the medically vulnerable and Migrant Seasonal Agricultural Worker populations in the six county area surrounding Lake Okeechobee, in Florida. FCHC is designated as a Federally Qualified Health Center (FQHC) and serves more than 55,000 patients. FCHC had no negative findings during its 2019 OSV. FCHC is Joint Commission (JC) accredited and a NCQA level 3 PCMH. Comprehensive care is provided through our network of 14 health centers. Counties we serve include: Glades, Hendry, Martin, Okeechobee, Palm Beach, and St. Lucie. Directlyâ€“provided services include Pediatrics, Family Medicine, Obstetrics/Gynecology (OB/GYN), and Dentistry, Infectious Disease Care, Pharmacy, Behavioral Health and SUD services. Target population served from this funding will be medically under-served, low income, Medicaid eligible, housing stressed, homeless, and linguistically isolated persons living in our six county region in Florida. Needs to be addressed: This project will address the need for comprehensive medical care (including prenatal) and dental care with particular attention to issues associated with access to care and Social Determinants of Health, like housing stress, diabetes, asthma, linguistic isolation, lack of dental care, and preterm birth. Also mental health and pharmacy needs. How project will address the need for comprehensive primary health care services in the community an d target population groups to be served: This project will continue to provide comprehensive medical care to the targeted population in the six county regions we serve. Quality clinical services are provided by qualified providers, referrals are coordinated through a team of care managers. Additionally, community collaboration is prioritized to ensure that patients are able to access other services needed for them and their family right in their community. Affordable services are provided to all patients regardless of their ability to pay. FCHC looks to provide services to those that would otherwise go without care due to the challenges mentioned above. FCHC is also offering telehealth services to eliminate transportation barriers for those patients that encounter transportation challenges. In addition, FCHC is now also offering services through a mobile health unit. The services are an extension of the health center offerings, but are provided in specific areas eliminating the need for patients to travel. The mobile health unit is an important mode of receiving services for those patients that live in rural areas. Proposed delivery sites and locations: All 14 FCHC sites are listed in the attached service delivery site document.
