Health Center Program - SAC ID:073 Health Center Program Award: H80CS00786 Family HealthCare Network (FHCN) was established in 1976 as a “Community Clinic” in Porterville in Tulare County, California. Its mission is “to provide quality health care to all.” Family HealthCare Network became a Federally-Qualified Health Center under Section 330 (H80CS00786) in 1984 and since then has opened 47 sites, of which 40 are clinical health center sites located in Cutler-Orosi, Farmersville (2), Goshen (3), Hanford (2), Ivanhoe, Pixley, Porterville (3), Springville, Terra Bella, Three Rivers, Traver, Tulare (3), Woodlake, Dinuba, Exeter, Reedley, Selma, Squaw Valley, Strathmore, Delano, Fresno (8), and Visalia (4). FHCN provides patient-centered services, including family and internal medicine, OB/GYN, pediatrics, dentistry, behavioral health, optometry, and 14 specialties. Other services include imaging, on-site laboratory, pharmacy, translation, transportation, health education, nutrition, patient outreach, and enrollment assistance programs. The service area population totals over a million people; more than 50% of residents live at or below 200% of the Federal Poverty Level (FPL). FHCN is accredited by The Joint Commission and recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home – both widely recognized symbols of quality and indicators that the medical practice is well-managed. As further evidence, in May 2024, FHCN received its HRSA Operational Site Visit without any conditions. In 2023, FHCN served as a “Health Home” for 238,703 patients, providing 1,103,541 patient visits: 95% of patients lived at 200% of the FPL, 75% were of Hispanic descent, 11% were uninsured, Medi-Cal covered 70%, 37% were dependent on the agricultural workforce, 31% were best served in a non-English language. FHCN is the safety-net provider for indigent, uninsured, migrant, seasonal farmworkers, and all persons with limited access (individuals at or below 200% FPL). Health statistics demonstrate that poor health access and health status exist in the context of rural communities with high rates of poverty, low educational attainment, immigrant populations, and non-English speakers. The low-wage agricultural industry has also left many residents with fewer resources to improve their health. Working collaboratively with the community, FHCN mobilizes combined energies to serve better patients and support initiatives that address health and access. The service delivery strategy includes full utilization of medical facilities, a diverse and competent workforce, expanded service hours, telehealth, health education, patient outreach, insurance enrollment, transportation assistance, and community collaboration to access available resources. With documented high unemployment, poverty, and uninsured figures far above national and State averages, there is little doubt about the need for the service area, SAC ID 073. This project addresses the goals of the Health Center Program by maintaining and expanding access to medical services for the uninsured and underinsured. In addition, the project supports growth initiatives and nurtures the infrastructure to maintain services. With continued operational support of $16,007,167 from the Bureau of Primary Health Care in 2025 - 2028, FHCN will assure continued access to affordable, quality primary health care services for 243,703 medically underserved and uninsured service area residents by the end of Calendar Year 2026.
