Health Center Program - Organization/community/target population: Established in 1970, Memphis Health Center, Inc., (MHC) is a Federally Qualified Community Health Center (FQHC) expanding access to care for medically underserved residents of portions of Fayette and Shelby counties, including the City of Memphis, in the southwest corner of Tennessee. In 1987, MHC initiated the first Healthcare for the Homeless (HCH) program in the city of Memphis. The proposed SAC will serve a total population of 311,133 in Shelby and Fayette Counties in Tennessee. Over 179,000 (57.57%) residents in the service area live with incomes below 200% Federal Poverty Guidelines (FPG), and 23% of service area residents lack insurance. The most recent 2022 Point-in-Time count identified 1,292 homeless living in the City of Memphis and Shelby County. The service area holds multiple health professional shortage area (HPSA) and medically underserved area (MUA) designations (HRSA Database). Health care needs/barriers: Residents of the service area suffer from high infant mortality rates at 9.4 (County Health Rankings, 2019) vs 5.6 at the national level (CDC WONDER, 2019). There is a significant and persistent public health crisis to the residents living in the service area demonstrated by the fact that the mortality rate from Diabetes, Cardiovascular disease and Colorectal cancer is above those of the national level at 29.1 vs 22.6, 242.1 vs. 194.0 and 21.2 vs. 16.2 respectively (CDC WONDER, 2019). Proposed project: MHC provides access to care in this high-need area through three freestanding primary care sites (two in Shelby County and one in Fayette County) and three homeless outreach locations co-located with area agencies serving the homeless population in Memphis. MHC provides culturally-competent, comprehensive primary, preventive, and supplemental health services to all people regardless of ability to pay. In 2023 51.19% of MHC patients lived with incomes below 200% FPG, and 29.23% were uninsured (UDS). Programs and Services Staff -WIC. MHC’s total target patient goal for 2026 is 18,414 patients with 54,181 total encounters (42,968 medical, 5,382 dental, 2,427 mental health, 1,839 substance abuse,1,565 Specialty (Podiatry and Vision) and 6,008 enabling Services encounters). MHC’s HCH program target goal for 2026 is 982 patients with 2,767 total encounters.
