Health Center Program - HEALTH CENTER CLUSTER - Kokua Kalihi Valley - Project Abstract Project Title: Service Area Competition Continuation Applicant Name: Kokua Kalihi Valley Comprehensive Family Services (KKV) Congressional District: HI-001 Funding Requested: $3,385,874 (CHC & PHPC) Continuation Grant. Funding Priority: Patient Trend & Patient-Centered Medical Home Recognition. KKVâ€™s SAC request aims to increase care access and reduce health disparities for predominately Native Hawaiian, Asian, and Pacific Islander patients living in the low-income community of Kalihi within the City of Honolulu, Hawaii. Established by community leaders as a 501(c)(3) in 1972 and as an FQHC in 1989, Kokua Kalihi Valley (KKV) has a mission to promote healing and reconciliation in Kalihi (zip code 96819). Home to 52,981 residents, KKVâ€™s service area is the most densely populated new immigrant community in Hawaii. Of the 10,693 patients served by KKV in 2020, 61.3% live at or below 100% of the federal poverty level and 30% who refused to state income level are presumed to be at or below 150% of the federal poverty level. KKVâ€™s patients are 36.5% Asian, 7.1% Native Hawaiian, 43% Pacific Islander, 3.2% mixed race, and 5% unknown. More than half (53%) are best served in languages other than English. The payment breakdown is 16% uninsured; 53.5% Medicaid; 4.2% dually eligible (Medicaid/Medicare); and 18.6% private insurance. Kalihi is both a Medically Underserved Area (MUA) and a Health Profession Shortage Area (HPSA) for Primary Care, Mental Health and Dental Care. Four public housing complexes are located within a one-mile radius of KKVâ€™s main clinic, including the first and third largest in the state. KKVâ€™s target population faces significant barriers to care including language, lack of insurance (Compact of Free Associ ation Micronesians were ineligible for Medicaid until 2021), and deep distrust of public and/or clinical care. These patients experience significant levels of chronic conditions such as diabetes (52.9%); hypertension (47.6%); and obesity. In 2020, KKVâ€™s Pacific Islander and Filipino patients were impacted by COVID-19 at the highest rates of any population in the State of Hawaii. KKV provides comprehensive, culturally responsive health care and social services in over 26 languages and dialects. Services are provided out of four clinical sites, with KKVâ€™s main clinic and wellness center both open 60 hours per week to ensure accessibility. As a 330(i) Public Housing Primary Care (PHPC) service provider, KKV maintains a satellite clinic at the Kuhio Park public housing development and has a multilingual staff providing outreach and transportation to assist residents from across the community. In the upcoming project period, KKVâ€™s 14.26 FTE medical providers, 7.5 FTE dental providers and 5.0 FTE behavioral health providers will serve an unduplicated 11,387 patients through 47,929 visits. KKVâ€™s service delivery sites include its main Health Clinic, the adjacent Wellness Center, the Gulick Elder Center, the Ke Ola Hou Clinic at Kuhio Park Terrace, and a new mobile clinic. Services include medical, dental, behavioral health, maternal and child health (perinatal, family planning, nutrition/WIC), immunizations, optometry, STD/HIV testing and services, health education, youth services, elder services, transportation, interpretation, outreach, eligibility, cultural education, and community advocacy.
