Health Center Program - Health Center Program award number: H80CS00729 The proposed service area identification number (ID), city, and state (available in the SAAT): 303, Portsmouth, VA The total number of unduplicated patients that you project to serve in calendar year 2026: 12,439 In response to a shortage and maldistribution of primary care physicians for an estimated 37,000 medically indigent residents, Portsmouth Community Health Center, Inc., dba Hampton Roads Community Health Center, was founded in 1995 and within a few short years transformed into the first and only Federally Qualified Health Center (FQHC) in Portsmouth, Virginia, a coastal city approximately three miles southwest of Norfolk, VA. At that time, two doctors, one family nurse practitioner, and eleven clinical support and administrative staff bolstered the efforts the founders had envisioned. The organization offers a comprehensive access point for low-income patients across six (6) counties with primary care, , mental health, dental, and enabling services through five clinic sites, one mobile medical van, and two mobile dental vans. In 2023, HRCHC served a total of 12,439 unduplicated patients across Chesapeake, Hampton, Norfolk, Portsmouth, Suffolk, and Virginia Beach – known as the Hampton Roads region. HRCHC’s mission is “We Care So You Can Be Healthy”, which illustrates the Center’s dedication to improving the lives of all people in the communities they serve. The overall purpose of the Center is to improve 1) access to quality and comprehensive health care services and 2) the health status of their patients through prevention, early identification, and treatment of serious health issues. HRCHC provides health care services to everyone while specifically targeting medically uninsured (homeless, public housing), and underinsured residents in the service area. HRCHC’s service area is home to 307,340 residents who experience low income. The penetration of low-income is 6.9%, indicating that more than 93% of low-income residents (286,243 individuals) are not served by a health center. The landscape of insurance coverage for the Center’s service area reveals employer-sponsored insurance coverage is lower than the state and nation in all 6 counties, while the reliance on Medicaid is significant – exceeding both state and national rates – in 3 counties. Portions of the Center’s service area experience a significant shortage of health care providers (primary care physicians, dentists, and mental health providers) relative to the population. Residents who identify as Black represent the primary minority in all counties and around half of the population in Hampton and Portsmouth. In fact, except for Virginia Beach City, the Black population is significantly higher when compared to the state, and up to more than four times that of the nation. Health disparities experienced by the Black population, often rooted in systemic inequities and historical injustices, highlight the need for culturally relevant care to improve health outcomes for the service area. In response to the service area’s significant and complex needs, HRCHC currently provides 138 hours of comprehensive health care services through their five clinical locations and 70 hours through their medical and dental mobile vans for a total of 208 hours to the six-county region.
