Health Center Program - Noble Community Clinics (Noble Clinics), a Federally Qualified Health Center funded under Health Center Program grant H80CS00713, has been serving the community since the 1960s. Its mission is to ensure access to high-quality, integrated healthcare and promote the well-being of our patients, staff, and communities. Under the proposed SAC ID 223 for Wautoma, WI, Noble Clinics aims to continue this mission. Established in 1972, it received Migrant Health Center (MHC) funding in 1973 and remains the only MHC in Wisconsin. In the late 1980s, Noble Clinics secured Community Health Center (CHC) funding and became a year-round FQHC, broadening its reach to include medically underserved populations in central Wisconsin. Today's service area covers all or parts of 17 counties in Central Wisconsin. As Wisconsin's only MHC, Noble Clinics serves Migrant and Seasonal Agricultural Workers (MSAW) across the entire state. The largely rural region includes small population centers, Stevens Point with 26,000 residents being the largest. The area also features Amish communities, most of which are served by Noble Clinics. Recognized for its strong regional partnerships and collaborative approach, they are a vital part of the region’s healthcare safety net. Noble Clinics operates seven fixed sites, one mobile unit, and two administrative locations. The headquarters in Wautoma, using a Patient-Centered Medical Home model, offers primary medical, dental, and behavioral health services, as well as 340B pharmacy and substance abuse recovery. The Friendship site, 33 miles west of Wautoma, provides behavioral health, substance abuse recovery, and primary medical care. A dental clinic is located 20 miles southwest of there at the Mauston location. In Beaver Dam, 40 miles southeast of Wautoma, there are two sites: one for dental and the other for primary medical and behavioral health. Stevens Point, 35 miles north of Wautoma, hosts two sites: one for dental and the other for primary medical and behavioral health. Noble Clinics' Mobile Medical Unit delivers primary medical services to MSAW at camps, fields, and canneries across Wisconsin, including medical and dental exams for children at Migrant Head Start locations. Most mobile services occur from May through August, with some year-round activities. Due to COVID-19, services for MSAW and their employers expanded to include vaccinations, testing, and education. Additionally, Noble Clinics provides school-based dental services at 65 sites in multiple counties and runs WIC programs in Waushara, Marquette, and Green Lake Counties. Based on 2023 UDS data, 14% of patients are uninsured, 93% are below 200% of the poverty level, and more than 15% are best served in a language other than English. Patients commonly face conditions like cardiovascular disease, hypertension, diabetes, obesity, asthma, behavioral health issues, and oral health problems. Noble Clinics served 17,088 unduplicated patients through 53,008 visits in 2023, along with 4,507 children through 9,598 visits in the school-based dental sealant program. Noble Clinics will use the Service Area Competition grant to continue offering integrated primary medical, dental, behavioral health, substance abuse recovery, and pharmacy services to the underserved and vulnerable populations of this region. Since 2016, Noble Clinics has more than doubled its patient base and encounters by strategically expanding with four new clinic sites, two administrative locations, and a state-of-the-art 40-foot Mobile Health Center. A recent remodel of the Wautoma site introduced 340B Pharmacy and complementary integrative medical services. They are currently constructing new comprehensive healthcare facilities in Stevens Point and Beaver Dam to expand medical, dental, behavioral health, and pharmacy services, further meeting the needs of these communities and surrounding areas. The projected number of unduplicated patients served in 2026 is 17,228.
