Health Center Program - HEALTH CENTER CLUSTER - Project Title: Service Area Competition Applicant Organization Name: Primary Health Network Website: (https://primary-health.net) Grant Program Funds Requested: Community Health Center (CHC) Proposed Service Area ID, City, and State: ID# 129, Sharon, PA Total Number of Unduplicated Patients Projected to be served in CY 2025: 87,000 Primary Health Network (PHN) was founded in 1984 with one small site and one part-time Provider. Today, PHN has grown to become the largest Federally Qualified Health Center (FQHC) in Pennsylvania, with over 40 sites spanning 17 counties throughout PA and eastern OH. In 2022, PHN served over 76,000 patients. PHNâ€™s mission is to, â€œâ€¦provides quality primary care services and access to specialty care commensurate with the needs of the people in the communities we serve. Services are offered regardless of age, race, creed, sex, national origin or ability to pay.â€ This mission is upheld by a strong leadership team, as well as a volunteer Board of Directors, composed of members who live and work throughout PHNâ€™s service area. PHN does not receive special population funding, and its service area includes both rural and urban communities. The target population, individuals below 200% FPL, is especially vulnerable, and PHN is often the only safety net provider for the counties wherein PHN is located. Throughout PHNâ€™s service area, specific needs vary, based on the community being served. Given the target population, economic distress and lack of access to care are common in each county served. Additionally, several needs are prevalent as common trends in the majority of PHNâ€™s service area. For example, over half of the counties served identified heart disease and cancer as the leading causes of death while most identified obesity and diabetes as a top priority. Also, nearly all the counties identified substance abuse and the need for mental health as top needs/concerns in their local Community Health Needs Assessments. PHN provides access to comprehensive health care services to all individuals, tailored to each community, by providing Primary Care Services (Family Medicine, Internal Medicine, Pediatrics, and Obstetrics/Gynecology), as well as Additional and Specialty Services (Behavioral Health, Psychiatry, Dentistry, Podiatry, and other services that are responsive to needs within PHNâ€™s service area). PHN also offers enabling services, which include free transportation, translation services, access to discounted medications, care coordination, and patient education. This is enhanced by Community Health Workers, who are actively engaged within PHNâ€™s communities to identify and work to resolve barriers related to healthcare and Social Determinants of Health. PHN continues to adapt with the changing healthcare environment by: maintaining noteworthy designations, such as accreditation with the Joint Commission and recognition as a Primary Care Medical Home; implementing new quality initiatives to close the gaps in care, such as the Collaborative Care Model; and partnering with communities to discover and eliminate barriers between residents and access to care, such as outreach and engagement via Community Health Workers.
