CHS expanded and has offered full-service community healthcare since 1978. CHS operates eight full-time 330(e) Community Health Center sites serving primarily six counties in northwest Ohio: Sandusky, Henry, Ottawa, Seneca, Wood, a small portion of Hancock county, plus a seasonal migrant-only site in Huron County. Overall CHS’ target population consists of the individuals with incomes at or below 200% of the federal poverty level (FPL), those who have Medicaid, Medicare, are uninsured or underinsured, and migrant and seasonal farmworkers. CHS also provides care for migrant and seasonal farmworkers through a small voucher program in cooperation with a farmworker program in eastern Ohio. The CHS service area faces health disparities and barriers to care related to shortages in primary care resources and unhealthy lifestyles. Chronic disease, including diabetes and cancer among others, as well as mental health issues, serve as the key issues faced by residents as well as migrant/seasonal farmworker populations. A number of factors contribute including an overall lack of access to health care, a large low-income population, poor diet, tobacco use/smoking and a lack of physical activity. The risk factors can appear early as young children and adolescents lack physical exercise, have poor diets, become overweight or obese, and learn other poor health habits. CHS addresses the need for comprehensive primary health care services, including dental and behavior health in the community and target population with outreach and education, expanded services and availability and increasing provider productivity. CHS fulfills an important healthcare need in its service area and has continued to build a strong comprehensive healthcare delivery system. With staffing plan of 105.40 FTEs including 12.4 medical providers, 11.6.0 dental providers, and 4.8 behavior health providers, CHS plans to provide almost 45,000 encounters to 11,800 patients. This Service Area Competition application will continue to reduce access to care barriers and offer the full range of Medical, Dental and Behavior Health related services, including Primary Care, Dental and Behavior Health Services directly on site by CHS to the underserved in northeast Ohio.
