Health Center Program - HEALTH CENTER CLUSTER - Project Title: NeoMed Center Competing Continuation HRSA-24-070 Applicant Organization Name: NeoMed Center, Inc. Web-Site Address: www.neomedcenter.org Grant Funds Requested: $6,921,432.00 per year for 3 years. Overview of organization: NeoMed Center was incorporated as a HRSA 330 center in 1992 when it opened its main center in Gurabo, Puerto Rico (PR). It currently has eight (8) sites: The main center in Gurabo; one (1) clinic in San Lorenzo, Juncos, and Trujillo Alto; two (2) clinics in Aguas Buenas and clinic created through a 2019 NAP in Naguabo, PR. As per the 2018 UDS data, NeoMed served 37,311 patients, using 37.92 FTE Physicians, attending 117,890 clinical visits. The patient population was 43.3% male and 56.7%, female, with 24% being pediatric patients (n=8,853) and 76% adults (n=28,458). An 86% had income under 100% of the Federal Poverty Level (FPL), and 96% below 200% FPL. A very high 22% was uninsured, while 57% had the public Vital Health Plan (Medicaid); 11.4%, Medicare: and less than 10% had private insurance. This evidences NeoMedâ€™s service to primarily low income and uninsured populations, including 1,735 PHPC patients as a special population in Aguas Buenas and 2,653 HIV+ patients served under the RW Part C funded Program. NeoMed is certified at Level 2 as a PCMH by NQAC and has JACHO certification for ambulatory clinics, with a 24/7 ER in its main site, and lab, X-ray and pharmacy services with 340B discounts. OH services are offered at the main site and integrated BH (including substance use treatment) in 5 clinical sites. NeoMed is herein applying for a 3-year Competing Continuation grant under this SAC of HRSA. Target population: The total estimated population of the 6-municipality servi ce area of NeoMed Center is 230,675 based on US Census data for 2022, 98% Hispanic and 97% of PR origin, is targeted under this SAC application, emphasizing Medicaid/Medicare eligible and uninsured persons, in these mixed urban/rural areas with MUA and HPSA classification and transportation, poverty and other access barriers. There is a declining population trend since natural disasters (Hurricanes Maria and Irma in 2017 and Fiona in 2022) plus a severe economic crisis, with an increase in elderly and a decreasing birthrate. The service area has over 73% of its total population with incomes under 200% poverty level and an average 14% unemployment rate compared to the nearly 10% for PR as a whole in 2018. A latest Community Health Needs Assessment showed disproportionately high rates of chronic illness compared to National Benchmarks include mortality rates per Diabetes (35.7% and 26.6%); Hypertension (65.0% and 57.5%); Heart Disease (13.5% and 12.9%); Current Adult Asthma (29.0% and 32.2%). These are also the most prevalent conditions attended in the NeoMed clinics. In addition, there were disproportionate percentages of depression and adult permanent teeth loss. This evidences the need for ongoing integrated PC/MH/OH services for patients in all of the NeoMed clinics. How project will address needs: Integrated PC, BH and OH services will continue to be provided to address identified needs, with outreach to low-income residents of outlying sectors with limited access to services. Services are provided in regular hours, M-F with extended hours in Gurabo, San Lorenzo, Trujillo Alto and Juncos clinics using multidisciplinary teams. Number of proposed new patients and visits: In the 3-year period of 2024-2027, 49,000 are projected of adult and pediatric patients expected to be enrolled and served in 120,000 visits.
