Health Center Program - HEALTH CENTER CLUSTER - Project Title: HRSA 24â€”070 Competing Continuation Applicant Organization: Centro San Vicente Website: centrosanvicente.org Type of 330 Section Funding Requested: CHC 330â‚¬ and HCH 330(h) Total Federal Funding Requested $4,880,248 Congressional District â€“ TX-16 About the Organization: Centro San Vicente is a multi-site. 501c (3) nonprofit, federally qualified health center started by the Daughters in 1988 from the proceeds of the sale of Hotel-Dieu Medical Center, hence our affiliation with Ascension. The health center is accredited by the Joint Commission. Centro San Vicenteâ€™s services are primary care, dental care, primary care, behavioral health/psychiatry including medication assisted treatment. This includes offering basic laboratory and radiology services, health education interventions, transportation, and eligibility assistance. Care coordination is provided in the linking patients to specialty care, including admission to local hospitals. More importantly, the Center operates a robust class A and class D pharmacies that offers affordable medications, pharmaceutical clinical management services for diabetic and hypertensive patients. For the community at large, Centro San Vicente operates a Hepatitis C clinic, Sexual Health Clinic, Gender Affirming Clinic for hormone therapy, and a robust Pre-exposure Prophylaxis clinic that offers PrEp and nPEP non-occupational post prophylaxis for HIV prevention. Centro San Vicente also participates in the National Hypertension Control Initiative by operating a remote patient monitoring program for hypertension. Centro San Vicente is also an advocate for social justice and a voice promoting the cause of improving access to equitable health care for the medically underserved, low income and marginalized communitie s in El Paso County. Community and Target Population: Centro San Vicenteâ€™s service area is comprised of the following twenty (20) zip codes: 79907, 79915, 79936, 79928, 79927, 79949, 79901, 799025, 79935, 79930, 79903, 79904, 79902, 79936, 29938, 79953, 79926, and 79922. The target population is low come, medically underserved, migrants, the homeless, members of the LGBTQI community, uninsured and underinsured persons, all ages, and primarily residents of El Paso County. Proposed Project: The proposed HRSA primary care project will continue to provide all FQHC required primary care and preventive services, and additional services at its existing four health center sites. The Center proposed serving approximately 20, 570 persons in 2024.
