Health Center Program - BronxCare Health Integrated Services System, Inc. (BLISS Inc.) has directed operations with PHS Section 330 funds since 1996 and provides comprehensive primary care, preventive, and specialty healthcare. BLISS oversees healthcare services at clinical sites distributed throughout its service area that are operated by subrecipients, BronxCare Health System, and the BronxCare Dr. Martin Luther King, Jr. Health Center, providing primary medical care to adults and children; obstetrical care; comprehensive dental, mental health, and substance abuse services; optometry; rehabilitative therapies; nutrition; and a host of specialty and enabling services. The service area is home to 753,626 residents who are predominantly minority, 44.3% Black/African American and 56.6% Hispanic/Latino. BLISS is designated as a Federally Qualified Health Center primary care HPSA, as are all census tracts in its service area; the primary service area includes 20 geographic units or populations that are HPSAs or MUAs/Ps. The 2024 BLISS needs assessment indicated that communities served by BLISS are among the poorest in the nation, with 40% of individuals living below the federal poverty level and are characterized by high unemployment, non-English speaking households, and low education levels. Although progress has been made in reducing health disparities, substantial inequities and barriers to care remain for service area residents. According to the 2022 County Health Rankings, the Bronx ranked last among New York State's 62 counties for health factors (based on weighted scores for health behaviors, clinical care, social and economic factors, and the physical environment). Substantial inequalities and barriers to care remain among the people served by BLISS; chronic and infectious diseases remain significant causes of illness, disability, and death. The leading causes of premature death in the Bronx include cancer, heart disease, unintentional injury, and diabetes. Low-income adults and children bear a disproportionate burden of chronic disease, use emergency services more often for routine care, have poorer health status, and have higher premature mortality. Social determinants of health, including the social and economic factors affecting health status and the physical environment, also placed the Bronx last among New York State's counties. Health disparities by age, gender, race, and ethnicity are also apparent in the Bronx. BLISS Inc. is governed by a Board of Directors that oversees the operational, financial, and clinical performance of FQHC activities and participates in strategic planning actions to meet the community's health needs. Together, BLISS and its sub-recipient partners are the largest and most comprehensive providers of healthcare services in the south and central Bronx. In 2023, BLISS served 103,157 unique patients. BLISS is committed to providing high-quality, comprehensive preventive and primary care across the lifecycle to meet the unique needs of clients and at-risk and underserved populations in the community. The total number of unduplicated patients projected to be served in the calendar year 2025 is 105,220, and in 2026 is 107,324. During this project period, BLISS will expand the hours of the FQHC sites to increase access for underserved and vulnerable populations. The expansion will include additional evening and weekend hours within the network of FQHC sites. HRSA BPHC Section 330 funds are requested to allow BLISS to continue to fulfill its mission to increase access to primary care and continue to reduce health disparities in the community.
