SUPPORT REQUESTED Christian Community Health Centers (CCHC) is requesting $2,604,693 in funding from the Health Resources and Services Administration (HRAS), Service Area Competition (SAC) grant to continue to provide quality comprehensive primary care, oral health and mental health services for medically insufficiently catered to areas and populations in Chicago Far South and South Suburban communities. With this funding CCHC proposes serving 23,000 unduplicated patients by 12/31/2028 across our four clinic locations: 9718 S. Halsted Street, Chicago, Il 60619; 901 E Sibley Blvd. South Holland IL, 60473; 364 Torrence Ave, Calumet City, IL 60409; 2320 Thornton Lansing Rd, Lansing, IL 60438, 1701 West 87th Street, Chicago, Illinois 60620 and through our Mobile Health Van Services. Christian Community Health Center (CCHC) is a community-based 501(c)3, not-for-profit, FQHC with Level III NCQA PCMH recognition, licensed by IDHS-DASA to provide outpatient substance use disorder (SUD) treatment since 2009. CCHC is an urban Federally Qualified Health Center (FQHC) on Chicago’s Far South Side (Roseland) and South Suburbs (South Holland, Calumet City and Lansing) proposing to provide accessible, integrated primary care, behavioral health, oral health care, HIV and MAT services to at least 23,000 unduplicated patients by 2028, regardless of ability to pay. In addition to health care services, we will also provide comprehensive social services, including interim and permanent supportive housing. CCHC meets the needs of each special medically insufficiently catered to population. CCHC addresses needs related to the following: Comprehensive primary health care services, including in-scope nutrition, chronic disease management, preventive health education, cancer screening, and mental health services. CCHC provides patient support services, specifically services that enable patients to access health center services, health and nutrition education services, and case management and care coordination services that can address health-related social factors such as those related to food insecurity, housing insecurity, and financial strain. Proposed Plan to Address Need: This application addresses need to reach unduplicated medically insufficiently catered to populations and strengthen their access and utilization of primary preventive care and medical home. CCHC aims to deliver measurable community impact and evaluation of outcomes demonstrating costs savings, sustainable services and improved health systems. CCHC is designated in 2025 as a Health Professional Shortage Area (HPSA) with HPSA scores that highlight the need for healthcare services in the community as reflected by a primary care of 21, a dental care of 25, and a mental health of 20.
