Health Center Program - Community Health Centers of Western Kentucky, Inc. Health Center Program Award Number: H80CS00562 Service Area ID: 266 City/State: Greenville, Kentucky Number of Unduplicated Patients Projected for Calendar Year 2026: 14,634 Community Health Centers of Western Kentucky, Inc. (CHCWK) has provided primary care services as a 330-funded Federally Qualified Health Center (FQHC) since 1994. Beginning as a small single site health center, CHCWK has continuously added providers, sites, and services to respond to the need for health care in the rural, impoverished community. CHCWK has been highly successful at identifying and addressing the needs of the target population, including multiple large expansion projects. CHCWK operates a total of five sites strategically located throughout the three-county service area. Due to the continued demand for accessible services, we are in the process of constructing a sixth site (projected to open by the end of November). Population Group to be Served: The service area of CHCWK includes Muhlenberg, Logan, and Todd Counties, located in the western coal fields region of Kentucky, which is part of the rural Mississippi River Delta Region. Some of the poorest towns in the nation are located in the service area. CHCWK’s target population consists of the low-income and medically underserved residents of the service area. Needs to be Addressed: CHCWK’s rural service area faces a shortage of providers, particularly those who will see the uninsured or those covered by Medicaid. All three counties in the service area are federally designated as a Health Professional Shortage Area (HPSA) for primary medical, mental health, and dental care. The entire service area is also federally designated as a Medically Underserved Area. There are no other FQHCs in the service area; CHCWK is the only safety net provider for the majority of low-income service area residents. In addition to provider shortages, the target population suffers from difficult life circumstances, including high rates of poverty, high rates of uninsured, high unemployment rates, and geographic/transportation barriers to accessing services. Health disparities are prominent for the target population, and include high rates of overall mortality, premature death, child mortality, infant mortality, preventable hospitalizations, chronic disease (heart disease, diabetes, respiratory disease, etc.), cancer, limited preventive care services (vaccinations, screenings, well visits, etc.), obesity, smoking, poor prenatal outcomes, metal health issues, substance use disorder, and poor oral health outcomes. These needs can be addressed through affordable, accessible access to health care and social support services, such as those provided by CHCWK. Proposed Services: With SAC funding, CHCWK will continue providing comprehensive and accessible health care services including medical, mental health, substance use disorder, dental, and enabling services at our six sites located throughout all three counties of the service area. Medical services will continue to be provided through our team of pediatricians, family nurse practitioners, family practice physicians, and an OB/GYN (also serves as our Medical Director). Our behavioral health providers will continue to include LCSWs, LPCCs, and psychiatric nurse practitioners. Enabling services will continue to be provided through a mixture of our case management, referral, and nursing staff. We will also continue our contract with a private local dentist to ensure access to this service. This level of clinical staffing will allow us to see 14,634 unduplicated patients with 63,007 visits (including all services) in calendar year 2026. As a fully operational FQHC with a deep understanding of the local community and experience in directly responding to the needs of the target population, CHCWK is well positioned to continue providing health care services in the three-county area.
