Health Center Program - HEALTH CENTER CLUSTER - Applicant Organization: Missoula County, Partnership Health Center Website: www.partnershiphealthcenter.com Grant Program Funds: $5,024,299 Patient Target: 19,199 Funding Priority: Patient Trend and PCMH Recognition Missoula County, Partnership Health Center (PHC) is pleased to present this proposal requesting $5,024,299 in funding under the HRSA 24-070 Service Area Competition. PHC is a department of Missoula County and the names Missoula County and PHC are used interchangeably throughout this application. For over thirty years, PHC has fulfilled our mission of promoting optimal health and wellbeing for all, through comprehensive, patient-focused, accessible, and equitable care. During this time, PHCâ€™s target population and services have continued to grow, with the support of Health Center program funding. PHC has received Patient Centered Medical Home (PCMH) recognition from the National Center for Quality Assurance, with a distinction in behavioral health integration, at our main in In calendar year 2025, PHC plans to serve 19,199 unique patients. This reflects an increase from our FY2020 target as well as a substantial increase from patient target listed in the service area announcement table. This increased target is in direct response to the growing need and population in our service area and PHC is well positioned to increase access with current levels of health center funding. According to the United States Census Bureau figures estimate Missoula Countyâ€™s population as 121,041, and the population of the two rural counties in our service area as: Ravalli, 47,298 and Mineral, 5,058. PHC is the largest federally qualified health center (FQHC) as well as the largest sliding-fee-scale provider of healthcare services in our 6,241 square mile service area. Other providers in the service area include one access point in Ravalli County, with limited provider access, and an Urban Indian Health Program, All Nations Health Center, an FQHC look-alike program in Missoula. As the primary care home for resident providers in Family Medicine Residency of Western Montana (FMRWM) is PHC is proud to help train the next generation of family medicine doctors. In addition to the main Creamery Building Clinic in downtown Missoula, Montana, PHC operates four satellite clinics across the region that provide fully integrated medical, dental and behavioral health services. These sites include a rural satellite clinic in Seeley Lake, Montana; an on-site clinic at Lowell Elementary School; a health care for the homeless clinic located at the Poverello Center, Missoulaâ€™s primary homeless shelter; and a clinic co-located at the Missoula Food Bank and Community Center. PHC has a growing behavioral health service line offering outpatient behavioral health services at the Alder Clinic location, and school based behavioral health at Willard Alternative High School and Lowell Elementary school. In the next year PHCâ€™s school-based program is expanding to C.S. Porter Middle School and Franklin Elementary school with the support of a HRSA school-based expansion award. PHC also operates the Ryan White Program, a supportive housing program with behavioral health and peer support services, a full-service pharmacy, clinical pharmacy, prenatal and high-risk OB services, a dentistry suite; care management, cancer control, and chronic disease self-management, as well as nurse care management for a variety of special populations. The grant funds requested here will support and expand the health center services described above as we continue to provide comprehensive health care to the medically underserved and special populations in our community.
