Exchange Ave., Chicago, IL 60617 HCP Grant # H80CS00514 Grant Program Funds Requested: CHC $7,062,465.00 Proposed Service Area: 335, Chicago, IL Patient Target: 28,929 Chicago Family Health Center (CFHC) is a Federally Qualified Health Center (FQHC) that provides care for the medically underserved on the South Side of Chicago. CFHC opened its doors in 1977 as a store front office where providers cared for less than 500 patients in the first year. For over 47 years CFHC has developed and grown to provide quality healthcare and support services for the medically overlooked and underinsured citizens of the South Side of Chicago. CFHC has grown to a network of six locations in South Chicago, Pullman, East Side, Roseland, Chicago Lawn, and Chicago Vocational Career Academy that has continued to provide a complete range of high-quality, integrated services, including: primary care, dental, behavioral health, and wraparound services, to more than 28,921 patients as of 2023 CFHC’s clinics are in some of the most medically underserved and economically depressed communities in Chicago. According to CFHC’s Uniform Data System (UDS) Report of 2023, 68% of low-income patients in CFHC’s service area accessed medical care last year from one of our health centers. Geo Navigator consistently maps CFHC’s ZIP code Tabulation Area (ZCTA) as the most underserved in the Chicagoland area. Accessing healthcare is challenging for uninsured and underinsured patients because accepting these patients is cost prohibitive to private physicians, many of whom have left the area and therefore increasing the need in these communities. All CFHC sites are in Healthcare Professional Shortage Areas as defined by HRSA. CFHC employs an integrated model of care designed to overcome barriers to care. CFHC’s model focuses on these specific modes of health care: primary care, oral health, and behavioral health. Primary care is the first aspect of CFHC’s care offerings; medical providers offer a wide range of services, including: pediatric care; women’s healthcare including OB/GYN; senior services and men’s healthcare. Dental services are the second aspect of CFHC’s care offerings; providers routinely perform cleanings, exams, x-rays, apply dental sealants, provide fillings, and provide oral health education. The third aspect of CFHC’s care offerings is behavioral health. Behavioral health providers commonly provide therapy for depression, anxiety, family and marriages issues, substance use disorder, medicated assisted recovery, group therapy for patients living with HIV\AIDS. smoking cessation, discipline issues for school-age children, domestic violence, and post-traumatic stress disorder. CFHC also offers access to psychiatry services. Other services offered, diagnostic labs, health screenings, pharmacy, health education, outreach, transportation, as well as programs such as WIC & the Illinois Breast and Cervical Cancer Program which offers cancer screenings free of charge. All of these and other wraparound support services such as case management, translation, care coordination, financial and insurance navigation and a sliding pay scale in accordance FPL guidelines helps CFHC provide whole-person and collaborative care to our patients. To support our efforts, CFHC has collaboration agreements with several local providers In 2023 CFHC’s UDS Report shows the following demographics: 28,921 unique patients, 53% living below 100% of the FPL. 68% qualify as low-to-moderate income. 73% rely on Medicaid, 5% on Medicare, 9% have private insurance, and 11% are uninsured. 53% are Black, 41% Latinx. 47% of CFHC patients identify as female. 26% identify as Male, 1% identify as Transgender, 3% refused to be identified. 4% self-identify as LBGTQ+, 61% self-identify as heterosexual, 35% refused to identify and 23% are best served in a language other than English.
