Health Center Program - Edward M. Kennedy Community Health Center Health Center Program Award number: H80CS00465 Proposed Service Area Identification number: 344, Worcester, Massachusetts The Edward M. Kennedy Community Health Center (Kennedy Community Health) was founded in 1972 by seven women, our seven founding mothers, who resided in the Great Brook Valley Public Housing in Worcester, Massachusetts, who wanted better health care for their families. Governed by patients and community members, Kennedy Community Health serves vulnerable and marginalized residents of our targeted communities of Worcester, Framingham, and Milford, and the surrounding areas in the Central and MetroWest areas of Massachusetts. Over 66% of our patients reside in our target communities, which are centralized within our service area and include the city of Worcester and the town of Milford in central Massachusetts (Worcester County) and the city of Framingham in MetroWest (Middlesex County). The entirety of our patient population encompasses 247 communities across our service area. Since our 2021 SAC application, Kennedy Community Health has actively expanded access to care to meet the growing demand in our service area. In 2023, we opened a second health center site in Worcester, located just half a mile from our flagship site and close to the Great Brook Valley Apartments and Curtis Apartments, which serve federal and state public housing families. Additionally, we inaugurated a larger health center across the street from our original site in Milford. This facility offers a comprehensive range of services, including medical, dental, behavioral health, enabling services, vision care, and a 340B pharmacy. With these additions, Kennedy Community Health now operates four state-of-the-art health centers, six school-based health center sites, and one administrative location, significantly enhancing our capacity to serve the community. Our patient population spans the globe and represents the changing demographics in our service area, including the country’s newest arrivals resettling in the Gateway City of Worcester. We have extensive interpreter services to deliver culturally and linguistically competent care in the 70 different languages spoken by our patient population; over 72% of our patients are best served in a language other than English. Approximately 90% of our patients are considered low-income using 200% of the federal poverty level, and a significant majority of our patients are uninsured or have Medicaid or other public health insurance at 29% and 54%, respectively. Roughly 2.1% of the patients we serve report that they are homeless, approximately 14% reside in public housing, and many more face housing insecurity. Kennedy Community Health is proud to be recognized as a patient-centered medical home by the National Committee for Quality Assurance (NCQA). We offer a comprehensive range of health care services, playing a vital role in the local health care delivery system. Our strong partnerships with local hospitals and referral networks ensure that specialty care is seamlessly integrated into each patient's overall care plan. As a beacon of equitable health care, Kennedy Community Health is committed to providing marginalized, vulnerable, and BIPOC populations with equal access to essential services. We actively reach out to these communities to enhance access to affordable health care. As our role in the communities we serve expands, funding from the HRSA 330 Grant will enable us to meet the growing health care needs of Central and MetroWest Massachusetts.
