Health Center Program - Project Title: Service Area Competition (HRSA-25-014) Applicant: Hamakua Health Center Program award number: H80CS00451 Address: 45-549 Plumeria Street Honokaa, Hawaii 96727 Proposed service area identification number: 121, Honokaa, Hawaii In 1966, five coastal plantation medical dispensaries located on the Island of Hawaii (also known as the Big Island), consolidated to form the Hamakua Infirmary. In 1993 the infirmary was transformed into Hamakua Health Center, Inc. (HHC), a community owned and governed IRS 50l(c)(3) nonprofit, Rural Health Center. Finally, in 2001, HHC was designated a Federally Qualified Health Center (FQHC). As a FQHC, HHC is committed to individual and community wellness by providing comprehensive, culturally competent, quality primary health care to medically under-served communities and vulnerable populations. This includes all people who face barriers in accessing services because of poverty, language or cultural differences, or an insufficient number of health professionals or resources in the area. HHC has locations in Honoka’a, Laupahoehoe, on the Hamakua Coast, Waimea, and in Kapa’au, on the Kohala Coast. HHC offers primary medical, dental, behavioral health, substance use disorder, cultural practitioner care, enabling services, and a variety of outreach programs to patients. No one is ever denied care or services, regardless of their ability to pay. HHC’s service area consists of 24 ZCTAs. The core of the service area (the areas in which 83% of health center patients reside) consists of 6 ZCTAs representing the communities of Honokaa, Paauilo, Kapaau, Kamuela, and Hawi. In a 2022 community survey, Hawaii’s high cost of living, especially for housing, healthcare, and food costs, contributed to economic insecurity. People reported that since the pandemic there has been a worsening of physical and mental health. In 2024 the Robert Wood Johnson Foundation research identified the areas in which Hawaii County is faring worse compared to the State of Hawaii and national averages, in the areas of teen births, low birth weights, available dentists, and more poor mental health days. In 2023 HHC served 8,051 unique patients. This included 24,623 combined in-clinic and virtual medical care visits, 1,696 dental visits, 3,571 mental health visits, and 308 substance use disorder visits. In 2023, 16% of HHC patients who reported income were living in poverty (at or below 100% of the Federal Poverty Guidelines (FPG)) with 39% below 200% of FPG (low income). Furthermore, 60% of HHC patients were recipients of public insurance with 4% having no insurance. This service to low-income residents substantiates the clinic’s role as a safety net provider to vulnerable populations in need of affordable and accessible care specific to their unique needs and circumstances. HHC requests funding in the amount of $1,776,492 to support the delivery of services to a patient population of no fewer than 8,051 unduplicated persons by the end of calendar year 2026. One hundred percent (100%) of funding is allocated to CHC funds.
