Health Care Needs/Barriers to be Addressed. Greater Meridian Health Clinic (GMHC), a federally funded community health center founded in 1986, provides primary and preventive health care for the medically underserved residents of Lauderdale, Noxubee, Kemper, Winston, and Oktibbeha counties in Mississippi. Over half (50.7%) of residents live with incomes below 200% FPG, 26.6% live with income below 100% FPG, 14.7% are uninsured, and 20.8% living with incomes below 200% FPG are uninsured (American Community Survey, Census Bureau, 2014-2018; SAHIE, Census Bureau, 2018).The population to 1.0 FTE primary care physician ratio in the service area is 7,415:1, much higher than the state (1,900:1) and national (638:1) ratios (County Health Rankings, Robert Wood Johnson Foundation, 2020; Americaâ€™s Health Rankings, United Health Foundation, 2018; HPSA Data, HRSA, 2020). These factors create significant barriers to care, which drive health indicators to exceed national and severe benchmarks. In 2020, GMHC served 8,690 patients with 19,994 medical, 2,798 dental, 52 mental health, and 134 enabling visits (UDS). Proposed Project and Services. GMHC operates nine free-standing sites and one mobile unit in the towns of Meridian, Shuqualak, DeKalb, Louisville, Scooba, and Starkville. Services include primary care; diagnostic labs and radiology; screenings; emergency and after-hours coverage; voluntary family planning; immunizations; well child services; OB/GYN including prenatal care, delivery, and postpartum care; dental care; pharmacy; case management; eligibility assistance; health education; outreach; transportation; translation; additional dental services; substance abuse and mental health services; physical therapy; occupational therapy; nutrition; and psychiatry. Year 1 proposed staffing includes 2.0 FTE family physicians, 0.75 FTE internist, 1.0 FTE OB/GYN, 1.0 FTE pediatrician, 9.0 FTE NPs, 1.0 FTE midwife, 20.0 FTE nurses, 2.5 FTE other medical staff, 2.5 FTE lab staff, 2.0 FTE x-ray staff, 6.0 FTE dentists, 1.0 FTE dental hygienists, 3.0 FTE dental assistants, 0.5 FTE LCSW, 0.35 LPC, 1.0 FTE BH Director, 0.5 FTE SUD providers, 2.0 FTE pharmacy staff, 1.0 FTE case manager, 2.0 FTE education specialists, 2.0 FTE outreach workers, 2.0 FTE QI staff, and 2.35 FTE other programs and services staff. From 01/01/2023-12/31/2023, GMHC projects serving 13,830 patients. Community/Target Population Groups to be Served. GMHC is requesting CHC funding to serve the low-income, underserved residents of its service area. All five counties hold Health Professional Shortage Area Designations (HPSA) for primary (low income in Oktibbeha), dental, and mental health care and are designated as Medically Underserved Areas (MUA) (Find Shortage Areas, HRSA, 2021). The service area is a mix of urban and rural communities, with a total population of 166,097. The total service area population is 52.0% White, 45.0% African American, 1.3% Asian, 0.4% American Indian/Alaskan Native, and 0.9% multiracial. Latinos comprise 1.6% of the population (American Community Survey, Census Bureau, 2014-2018).
