Health Center Program - B-K Health Center Inc. d/b/a NEPA Community Health Care Grant Number: H80CS00420 Service Area ID: 206, Hallstead, PA NEPA Community Health Care was established in 1983 as a non-profit 501(c)(3) federally qualified health center serving the people of rural Susquehanna County. To ensure that we are delivering the highest standards of care to our patients, NEPA Community Health Care sets a lofty standard of regulatory compliance, financial integrity and clinical quality. It is our core belief that primary care is the foundation of whole-person health and, as such, the gateway to all medical subspecialties. As medicine and technology continue to advance, family practice remains at the center of care coordination, relying upon the long-standing relationships built with patients to direct the course of appropriate and effective treatment. Our roots may be grounded in primary care, but we haven’t stopped there. NEPA Community Health Care has since expanded both centers and services throughout the region. In 2015, the introduction of mental and behavioral health service lines positioned NEPA Community Health Care as the county’s designated outpatient mental health provider. Shortly after, in January 2016, the Matthew W. Stahl Memorial Health Center became a reality for the Montrose community. Then, with tremendous support from donors, the Andrew C. Mazza Memorial Health Center opened its doors in April 2018, marking our entry into Lackawanna County. Finally, testament to the strength and longevity of our partnership with local donors, dental services were launched in March 2021. Most recently, collaboration and resources from Maternal & Family Health Services have assisted us in expanding obstetrics and gynecology services to our community. Our strategic growth from primary care to mental health, dental, and women’s health services depicts our effort to address the needs of patients within our service area. Our rural and predominantly low-income communities have struggled to access equitable care in the areas of dental, mental health, and women’s health. Until NEPA CHC expanded services and access, many residents of Susquehanna County chose not to seek care because it was not readily available close to home. Barriers to care such as transportation, childcare stood in the way of traveling 30-60 minutes from home to see a dentist or a gynecologist. As an organization invested in the health and well-being of our community, NEPA Community Health Care continues to strengthen its collaborative partnerships with agencies like Interfaith and Trehab, who played a critical role in the initiation of our medication-assisted treatment (MAT) service line in 2017. Since that time, NEPA Community Health Care has received designation as a Center of Excellence for Opioid Use Disorder through the Pennsylvania Department of Human Services. Centers of Excellence make sure every person with OUD achieves optimal health. This means they take care of the whole person, including physical health management (for conditions such as diabetes) and mental health treatment (such as anxiety or depression). It also means that COEs provide one-on-one support to every patient through a Care Management team which helps identify, organize, obtain and sustain treatment resources. We could not be more excited about the future of our organization and the countless ways there are to continue positively impacting our rural community while breaking down barriers to accessing care. We will continue to serve the 7,671 unique patients who have chosen us as their provider of choice. And continue to provide equitable health care services to all patients who seek it, regardless of their insurance coverage, ability to pay, language, ethnicity, or gender identity.
