HEALTH CENTER CLUSTER - Project Title: Service Area Competition Applicant Name: Southeast Alabama Rural Health Associates (SARHA) Web Site Address: www.sarhaonline.com Congressional districts within your service area: AL-2 Service Area Identification Number: 160 Current federal funding: HRSA Section 330 funding requested: CHC & MHC Southeast Alabama Rural Health Associates (SARHA), a private non-profit corporation based in Troy, Alabama requests continued federal support to begin its 39th year of operation as a comprehensive Community Health Center organization on April 1, 2022. SARHAâ€™s primary service area consists of all or portions of Barbour, Coffee, Crenshaw, Dale, Geneva, Henry, Houston & Pike Counties, & contiguous areas all located in Alabama Congressional District 2. The area is described as extremely rural with much of the area having characteristics similar to a third-world nation. Factors such as low income level, low educational attainment & diverse cultural systems cannot be ignored as major access barriers to an already overburdened & inadequate health care system. The health care needs of the area are deeply rooted in the health, economic, & cultural systems existing in southeast Alabama. The top three (3) diagnoses among SARHA users are hypertension, hyperlipidemia, & diabetes with heart disease, cancer & chronic lower respiratory disease being the three (3) leading causes of death among area residents. SARHA provides primary & preventive health services to all residents of the service area in need of a medical home without regard to their ability to pay. SARHA providers serve as the only source of health care in four (4) of SARHAâ€™s ten (10) service communities. Services include, but are not limited to, primary m edical care, OB/GYN care, diagnostic laboratory, radiology & tests/screens, urgent medical care, 24-hour coverage, hospitalization, voluntary family planning, HIV testing, immunizations, hearing & vision screening, pharmacy, health education, case management, eligibility assistance, discharge planning, outreach, mental health & substance abuse, specialty referral & translation. These services include screening, testing, & treatment for coronavirus, as well as the provision of the vaccine. Services are provided through the Abbeville Family Health Center, Clayton Family Health Center, Dothan Family Health Center, Dozier Family Health Center, Enterprise Childrenâ€™s Center & Family Medicine, Eufaula Family Health Center, Geneva Family Health Center, Newton Family Health Center, Slocomb Family Health Center, & Troy SARHA Doctors Center. SARHA serves both the general community & the migrant & seasonal farmworker (MSFW) populations. SARHA addresses the special needs of the MSFWs through culturally competent healthcare services that are accessible in the farmworkersâ€™ native language, aggressive outreach that is integrated into the primary health care delivery system, the use of lay health workers & health teams that travel to migrant fields, & education for farmworkers & clinicians. During calendar year 2020, SARHAâ€™s 11.27 FTE physicians & 12.83 FTE mid-level practitioners served 32,743 patients who made 96,340 medical visits. The 2022-2025 patient goal is 63,871. SARHA takes a care-coordinated, case managed approach to the delivery of services through an established, fully operational cooperative network of social & health service providers that is over 35 years old. SARHA became accredited by the Joint Commission for Ambulatory Care & Laboratory Services in 2006 &, in 2013, achieved certification by the Joint Commission for Primary Care Medical Home. SARHA underwent HRSAâ€™s Operational Site Visit in August of 2 019, through which the organization, again, demonstrated full (100%) compliance with all HRSA core requirements.
