(PCC), located in Franklin, West Virginia, provides high-quality primary care to residents throughout Pendleton, Grant, Hardy, Randolph, and Tucker Counties in West Virginia, and Highland County in Virginia. PCC operates several facilities, including three health centers in Franklin, Riverton, and Harman, WV; a nursing home, Pendleton Manor Nursing Home; and four school-based clinics located at Brandywine Elementary School, Franklin Elementary School, North Fork Elementary School, and Pendleton County Middle/High School. For residents who are unable to travel to a health center site, PCC also provides in-home services. The Center offers a wide range of services for patients, including laboratory services, diagnostic X-rays, screening mammograms, immunizations, voluntary family planning, mental health, after-hours emergency coverage, case management, health education, and community outreach. For services not provided directly by PCC, referrals are made to partner community organizations. All PCC services are available to patients of all ages regardless of their ability to pay. PCC works to provide services to residents within the Appalachian Mountain range, targeting those below 200% of the Federal Poverty Guidelines (FPG) regardless of their health insurance status. The health center’s service area is composed of individuals and families that are impacted by transportation, access to care, and poverty. Travel throughout the service area can be difficult due to winding, mountainous roadways, and public transportation options are limited. Residents have difficulty finding primary care providers, physician specialists, dentists, and mental health providers due to the significant shortage of providers throughout the service area. The scarcity of employment opportunities and low educational attainment have resulted in increased poverty in the service area. PCC’s goal is to increase access to primary care services to better serve the needs of the community. The inability to receive primary, preventive care services can frequently result in poor management of health conditions, especially chronic health conditions, often resulting in increased emergency room utilization. PCC is committed to continuing to assist patients in their ability to access essential primary care services, proactively managing the health and well-being of residents throughout the rural West Virginia service area.
