Founded in 1976, Health West has expanded from one clinic in Pocatello, ID to 18 comprehensive medical, one pediatric/OBGYN, five dental, seven pharmacy, & 11 school-based clinics in the towns of Aberdeen, American Falls, Chubbuck, Downey, Lava Hot Springs, Montpelier, Pocatello, & Preston, ID; Brigham City, Garden City, North Logan, & Providence, UT; & Evanston & Rock Springs, WY. Health West operates with over 320 clinical & administrative staff that provide primary care, urgent care, OBGYN, pediatrics, dental, behavioral health (BH), pharmacy & medication management, diagnostic lab & radiology, health education, case management, & enabling services both in-person & through telehealth, regardless of ability to pay. For over 48 years, Health West has worked with community leaders, other healthcare providers, & social service & community organizations to assess the healthcare needs & barriers to care of the region’s vulnerable populations & develop programs & services that address those needs. As a result, Health West has become an integral component of the region’s health delivery network providing a continuum of care that improves the health status of its patients. In 2023, Health West served nearly 29,600 unique users, providing 104,500 medical, dental, & BH encounters, & 1,400 enabling visits. Health West’s target population is medically vulnerable people living at or below 200% FPG, who are affected by socioeconomic, geographic, environmental, cultural & ethnic health disparities. In the service area 16% of the population lives in poverty, 40% lives below 200% FPG, 21% are uninsured & 25% are on Medicaid, 28% of adults are obese, 9% have diabetes, 16% have hypertension, & there is a high number of adults who have not accessed primary care due to cost. This demographic is reflected in the health center’s current users, of which a significant number have been diagnosed with chronic disorders. Health West also provides access to nearly 11,200 privately insured individuals living in remote, rural areas within the 10-county service area who would otherwise not have access to affordable health care services. Health West continually strives for quality improvement, which will be a primary focus over the next project period. Health West will strengthen its pro-active, community & evidence-based model of care to help patients achieve optimal health outcomes. Efforts will focus on decreasing disparities in access to prenatal, pediatric, BH & preventative care for those at risk of cancer, heart disease or diabetes. Outreach/enrollment specialists will assist in the Medicaid, Medicare & private Marketplace insurance application process ensuring that patients have access to services. The 340B pharmacy program will continue to provide uninsured & indigent patients with access to needed medication, & outreach programs will be expanded to meet the changing needs of the service area population. Health West will continue to serve as an educational resource for local training programs including family practice residents, physician assistants & pharmacy students. In the upcoming project period, Health West expects to reset the patient target to approximately 28,124 unique users
