Health Center Program - Community Health Centers of Burlington, Inc. Health Center Program Grant #: H80CS00311 SAC ID#: 304, Burlington, VT Projected # of Unduplicated Patients in CY 2026: 31,038 Community Health Centers of Burlington (CHC) is a 53-year-old health and human service institution that is the sole FQHC available to approximately 26% of Vermont’s population that resides within our service area of Chittenden County and southern Grand Isle County. Our history of federal grant support began in 1988, with Vermont’s first Health Care for the Homeless (HCH) grant. Designated an FQHC in 1994, CHC has evolved into the leading safety net provider within our medically underserved area and the region, serving the state’s largest pocket of homeless and uninsured community residents. With a focus on providing care to all within our community, CHC has become the largest provider of health services to the homeless in Vermont and other marginalized communities and remains the sole statewide grantee of HCH funding. CHC patient needs mirror typical urban social problems such as generational poverty, addiction, mental illness, poor oral health and hygiene, refugee status and homelessness. The majority of our patients share a common struggle with marginal incomes, with approximately 40% living at or below 200% of poverty. In Calendar Year 2023, CHC served 29,998 patients with 110,184 visits (HRSA UDS), and projects 31,038 patients with 132,584 visits by the end of Calendar Year 2026 (including all visits, both UDS and non-UDS). Our proposed project addresses community health needs by providing easily accessible sites and comprehensive services designed to eliminate health disparities and barriers to care. Our services are delivered across nine sites in a warm, respectful, culturally competent manner that speaks to our values and commitment to remaining Vermont’s premier FQHC. CHC’s services include primary and preventive medical care, including gender-affirming care; restorative and preventive dental care; obstetrical and prenatal care; mental health; addiction and substance use treatment; financial assistance through our Sliding-Fee Scale; interpreter assistance for our large refugee and immigrant population; insurance screening and application assistance; a new in-house pharmacy (2023) with connection to low-cost prescription medication programs; psychiatry, including a unique same-day appointment model to ensure access to CHC patients in crisis, and wrap-around services including case management; Doctor-at-Home services; a newly expanded in-house laboratory (2024); and robust outreach and housing programs for homeless individuals, including a van for meeting this patient population “where they’re at.” CHC has also seen additional growth since our last service area competition submission: In 2022, we successfully opened our ninth CHC practice location in Essex Junction, Vermont, and in 2023, added dental services at our CHC – Winooski practice location through the integration of Vermont Dental Care, a formerly independent dental practice. Additionally, with funds received through Congressionally Directed Spending, we completed construction on a new dental center at our CHC – Champlain Islands practice location, which we expect to open shortly. CHC also hired its first Director of Diversity, Equity, and Inclusion in 2022 to enhance staff members’ cultural competence in working with our diverse patient population, and to build collaborations with various community partners that serve the same populations.
