Health Center Program - Brief Overview of the Organization, Community to be Served, and Target Population: Project Renewal, Inc. (PRI) submits this application in response to the HRSA Service Area Competition (SAC) funding opportunity for service area ID#043, in New York, NY. PRI’s mission is to end the cycle of homelessness by empowering clients to renew their lives with health, homes, and jobs. Beginning as a pioneer in 1967 with a 48-bed alcohol detoxification program on the Bowery, and today, through three main service lines (housing, employment services, and health care). Since its founding, PRI has consistently spearheaded new and award-winning approaches to serving this marginalized population, including: the launch of supportive housing for New Yorkers in addiction recovery; the integration of occupational therapy into shelter-based services; opening a non-hospital-based clinic for medical detox and follow-up care; and debuting a shelter designed specifically for LGTBQIA+ young adults, among many other firsts. To enhance the healthcare services delivered, PRI became an FQHC in 2002. As NYC homelessness grows—especially among older single adults, who comprise most of our clientele—PRI continues to innovate in the face of this crisis, but our vision remains the same: to respond to homelessness with creative, comprehensive, and compassionate responses. Need for Comprehensive Primary Care Services in the Community and Target Population: Nearly all patients served by PRI are homeless, and many contend with or have histories of mental illness, substance use disorders, justice involvement, and/or chronic physical medical conditions. PRI serves a diverse patient group (49% Black or African American and 30% Hispanic or Latino) and many are older adults that require specialized care needs responsive to their unstable housing and/or chronic conditions (45% of patients are 50 or older). Almost all patients are unemployed, underemployed, uninsured, and/or underinsured, living below 200% of FPL (87%). Homelessness and health are inextricably linked: furthermore, fragmented healthcare delivery systems, lack of health insurance, limited resources, and the prioritization of basic survival needs regularly prevent this population from seeking care except in emergencies. Research consistently demonstrates that homeless individuals exhibit higher rates of disease, reduced access to treatment, and poorer prognoses as compared to the housed population. Number of Proposed New Patients, Visits, Providers, and Services: PRI proposes to continue provision of outpatient primary care, mental health, substance use disorder services, and oral health care, as well as a range of supportive and enabling services to improve health outcomes for individuals at risk for or experiencing homelessness across all five boroughs of NYC. These services will be offered through PRI’s five shelter-based integrated care clinics and three medical vans that provide medical care at homeless shelters and other locations accessible to the targeted homeless population. In the upcoming project period, PRI anticipates serving 3,267 patients by the end of calendar year 2025.
