Health Center Program - Service Area ID #: 134, Franklin, NH Total # of unduplicated Patients to be served in calendar year 2026: 7,497 Congressional Districts Impacted: NH-001 & NH-002 The Organization: HealthFirst Family Care Center (HFFCC) is an independent, non-profit established in 1996 whose mission is to provide high-quality primary healthcare, integrated behavioral health, treatment, prevention, and education services to residents of our service area, regardless of their ability to pay or insurance status, depending upon available resources. To coordinate and cooperate with regional healthcare and specialty providers to assure the people of this community the fullest possible range of healthcare and prevention services. HFFCC was created to make quality and compassionate primary healthcare readily available to area residents. We offer complete, coordinated, and individualized healthcare to people of all ages; from babies to great-grandparents. HFFCC is committed to the delivery of a coordinated, personalized care experience and this designation reflects the hard work and dedication of HFFCC staff who truly care about our patients and improving their health outcomes. HFFCC also fosters strong collaborative relationships with a wide variety of public and private entities within its service area and the State to support access to health care services in its effort to promote a healthier community and population. Service Area & Target Population: HFFCC provides services at sites in Laconia, Franklin, and Canaan, NH and proudly serves twenty-eight rural townships: Alexandria, Andover, Ashland, Belmont, Bridgewater, Bristol, Canaan, Center Harbor, Danbury, Dorchester, Enfield, Franklin, Gilford, Gilmanton, Grafton, Groton, Hebron, Hill, Laconia, Meredith, Moultonborough, New Hampton, Northfield, Orange, Salisbury, Sanbornton, Sandwich, and Tilton. The majority of the communities are designated as Medically Underserved Areas or Populations. The target population includes the 24,325 low-income residents of the service area. Census estimates reported in the Health Center Program GeoCare Navigator indicate 8.8% of service area residents are living in poverty with household incomes less than 100% of the federal poverty level (FPL) compared to 7.4% of New Hampshire residents. Additionally, 21.4% of service area residents, as opposed to 18% of New Hampshire residents, were considered low income (<200% of FPL). Health Care Needs & Barriers to be addressed: Needs identified in the 2020 community health needs assessment: 1) affordable health insurance/health care cost; 2) Availability of mental health care services; 3) cost of prescription drugs; 4) availability of primary care services; and 5) availability of substance misuse treatment and recovery services. Economic status, including access livable wages, affordable housing, and affordable health insurance; all present significant barriers to care in the HFFCC Service Area. Additionally, the economic status of nearly 24% of the residents as low income often limits access to primary care. Project Summary & Services Provided: HFFCC proposes to address the need for comprehensive primary health care services to the target population through its integrated delivery model offered on a sliding fee scale to those who are low-income. Services provided include: primary medical care, substance use disorder treatment; behavioral health; preventive oral health; comprehensive care management; enabling services; pharmacy assistance; and health insurance marketplace outreach and enrollment. Locations: 3 full-time offices: Franklin (50 hrs./week), Laconia, NH (50 hrs./week), Canaan, NH (40 hrs./week). Total number of unduplicated patients projected to be served in 2026: 7,497
